Tetralogy of fallot with pulmonary atresia and aorto-pulmonary window: Or is it truncus arteriosus?

A four month old female child presented with past history of multiple episodes of respiratory tract infection. On examination, she was febrile, tachypneic and cyanosed. Chest radiograph showed normal cardiothoracic ratio with pulmonary plethora and mediastinal shift to the right. She was maintaining low oxygen saturation even with oxygen support.
