Postpartum Hemoptysis as Presenting Sign of Longstanding Vasculitis

We present the case of a 26-year-old patient who presented to our hospital in labor. Her medical history included suspected latent tuberculosis diagnosed on a prior chest x-ray and a positive tuberculosis skin test, after which she had undergone treatment with isoniazid. She was unable to recall whether she had received a tuberculosis vaccine as a child when living in Myanmar. Throughout the course of her pregnancy she had persistent microscopic hematuria and proteinuria, of unclear etiology. She was followed in our resident obstetrics clinic, and was evaluated by the nephrology team with plans for a postpartum renal biopsy.
