Post-traumatic psychiatric disorders: PTSD is not the only diagnosis. Presse Med. 2018 Mar 23;: Authors: Auxéméry Y Abstract Traumatic events and their consequences are often hidden or minimised by patients for reasons linked to the post-traumatic stress disorder itself (inexpressibility, shame, depressive thoughts, fear of stigmatisation, etc.). Although post-traumatic stress disorder (PTSD) remains the most widely known disorder, chronic post-traumatic psychiatric disorders are many and varied. After a trauma, the practitioner has to check for the different clinical forms of post-traumatic psychological consequences: PTSD is not the only diagnosis. Based on our own clinical experience compared to the international literature, we think necessary to build a didactic classification describing chronic post-traumatic symptoms and syndromes. Post traumatic depressions and bereavement lead to high risk of suicidal crisis and self-harm behaviours. Re-experiencing are felt with anxiety, hyper arousal increases anxious reactivity, and avoidance strategies increase anticipatory anxiety, indicating post-traumatic anxiety disorders (agoraphobia, specific phobia, obsessive compulsive disorder, separation anxiety, social phobia). Conclusion: Experiencing defibrillator shocks was associated with psychological distress in ICD recipients; the relationship was fully mediated by ICD-related concerns and perceived control. These results suggest that clinicians should routinely assess ICD-related concerns and perceived control in patients with ICD. Research is needed to develop and test interventions to decrease emotional distress related to the ICD shock experience.

Conclusions: Knowledge of HIV status, be it positive or negative, was significantly associated with a decrease in depressive symptoms in 2 FSW populations. The expansion of HIV testing programs may have mental health benefits for FSWs.

DiscussionThese preliminary results suggest that even if patients believe that they have reduced cognition caused by their vestibular deficits, it remains challenging to demonstrate the effects using simple (standard) objective cognitive measures. We discuss the mismatch in results in terms of cognitive specificity, complexity and compensation. We propose that the objective measures may more likely show effects if combined with a dual task, or by performing complex imagery tasks, adding strain to cognitive resources.

Discussion/conclusionNous démontrons dans cette étude un phénotype posturo-locomoteur typique d’une atteinte vestibulaire unilatérale. Ce test supplémentaire permet de renforcer les outils diagnostiques de la pathologie sur le modèle rongeur de lésion vestibulaire unilatérale mais aussi de tester des composés pharmacologiques favorisant la restauration de l’équilibre. L’utilisation de ce dispositif dans le domaine clinique pourrait avoir des avantages dans le diagnostic et dans le suivi de la réhabilitation pharmacologique et physique du patient.

Discussion/conclusionIl existe une différence en termes de stabilité entre ces deux positions, et la position naturelle la plus stable pour les sujets s’organise de manière plus précise et plus sélective malgré un polygone de sustentation que peu modifié. En termes d’analyse de sujets pathologiques, cela semble déterminant pour une prise en charge thérapeutique appropriée et des exercices rééducatifs bien contrôlés.

DiscussionIndépendamment des sollicitations sensorielles, les sujets hémiparétiques droits présentent des stratégies adaptatives différentes des sujets hémiparétiques gauches. Une masse ajoutée, ou une modification du champ visuel favorise ou désavantage l’équilibre assis avec une accentuation des effets sur les plans de contrôle de la stabilité.
