Single-center Series of Bone Marrow Biopsy-Defined Large Granular Lymphocyte Leukemia: High Rates of Sustained Response to Oral Methotrexate

Conclusion Single-agent oral methotrexate appears to be highly efficacious, resulting in long response durations and minimal toxicity. Micro-Abstract A total of 39 patients were diagnosed with large granular lymphocyte leukemia using stringent diagnostic criteria. Of the 39 patients, 15 (38%) were untreated. Of the remaining 24 patients, 13 were initially treated with prednisolone. The overall response rate (ORR) was 84.6%, and the median duration of response (DOR) was 13.5 months. For the 9 patients who received oral methotrexate, the ORR was 89% and the median DOR was 132.7 months. Five patients received methotrexate after prednisolone failure, with a treatment response in all 5. Single-agent oral methotrexate results in long responses with minimal toxicity.
