Endoscopic ultrasound –directed transgastric ERCP for management of severe hepatic steatosis and fibrosis in a liver transplantation patient with bypass anatomy

A 54-year-old woman who had previously undergone Roux-en-Y gastric bypass presented with cholangitis 1 month after liver transplantation for alcohol-associated cirrhosis. Percutaneous transhepatic biliary drainage (PTBD) was performed for management of a suspected anastomotic stricture. Subsequent capping trials were unsuccessful, and the patient underwent placement of an internal-external percutaneous drain for several months, averaging 600 mL of bile drainage daily.
