Comparison Of Pharmacist-directed And Protocol-driven Warfarin Management In Ambulatory Patients With Left Ventricular Assist Device
Management of advanced heart failure patients with a left ventricular assist device (LVAD) improves survival and quality of life. Patients with an LVAD require anticoagulation with warfarin for thrombosis prevention. Complications observed include both bleeding and thrombotic events (BE and TE, respectively). Median time in therapeutic range (TTR) reported in clinical studies for other disease states is 65%. Limited studies have evaluated warfarin management in the outpatient setting for patients with an LVAD.
