Aortic Valve Injury During Thoracic Endovascular Aortic Repair Requiring Emergent Surgical Aortic Valve Replacement
Endovascular management has become the preferred therapy for anatomically appropriate descending thoracic aortic aneurysms due to decreased rates of adverse cardiac and pulmonary events, decreased rates of kidney injury, shorter length of stay, and similar long-term outcomes when compared to open techniques.1,2 Because the surgical procedure requires large-bore (>20F) arterial access and the passage of catheters and stiff wires into the aorta, patients are at risk for hemorrhagic, embolic, and direct mechanical complications related to aortic instrumentation.
