Cardiac Ct Evaluation Of Post-infarction Intramyocardial Fat: A Critical Marker Of Adverse Lv Remodeling

Introduction: Post-infarction intramyocardial fat (PIF) is a relatively rare finding on cardiac computed tomography (CT) imaging but has been increasingly recognized as an indicator of poor prognosis in patients with ischemic heart disease. PIF appears as areas of low attenuation within the myocardium and has been associated with a higher risk of adverse cardiovascular events, heart failure and sudden cardiac death especially in non-ischemic cardiomyopathy. Recently, intramyocardial fat has been associated with fatal tachyarrhythmias in post-reperfused ischemic heart disease.
