I'm currently working at a dermatology office with one dermatologist where every visit, even 5 minute encounters, is billed as a 99214. Since I started working here, I've seen maybe one visit billed as 99213. From my experience at other offices, this appears to toe the line for unethical behavior, but I wanted some opinions from attendings on whether this would be constituted as upcoding.
