Cryotherapy following total knee replacement
CONCLUSIONS: The certainty of evidence was low for blood loss, pain and range of motion, and very low for transfusion rate, function, total adverse events and withdrawals from adverse events. We are uncertain whether cryotherapy improves transfusion rate, function, total adverse events or withdrawals from adverse events. We downgraded evidence for bias, indirectness, imprecision and inconsistency. Hence, the potential benefits of cryotherapy on blood loss, pain and range of motion may be too small to justify its use. More well-designed randomised controlled trials focusing especially on clinically meaningful outcomes, such...

Trauma-Informed Care in Medical Imaging and Radiation Therapy to Reduce Retraumatization
CONCLUSION: Because many aspects of care, including routine care in medical imaging and radiation therapy, can be an unintentional reminder of a traumatic experience, health care professionals should be trauma-informed when they are interacting with and caring for patients.PMID:37709517

Hesperidin: Enrichment, forced degradation, and structural elucidation of potential degradation products using spectral techniques
CONCLUSIONS: Hesperidin was labile under acidic and basic stress conditions. The potential DPs were characterized using LC-ESI-MS/MS and NMR spectral techniques. The proposed mechanism of formation was hypothesized. In addition, to identify and characterize the DPs, a SIAM, which has broad biomedical applications, was successfully developed.PMID:37706431 | DOI:10.1002/rcm.9615

Cancers, Vol. 15, Pages 4558: Indirect Treatment Comparison of First-Line CDK4/6-Inhibitors in Post-Menopausal Patients with HR+/HER2 & minus; Metastatic Breast Cancer
Conclusions: Findings from this indirect treatment comparison suggest no significant PFS or OS differences between CDK4/6-inhibitors in post-menopausal patients with HR+/HER- MBC.

Higher versus lower fractions of inspired oxygen or targets of arterial oxygenation for adults admitted to the intensive care unit
CONCLUSIONS: In adult ICU patients, it is still not possible to draw clear conclusions about the effects of higher versus lower oxygenation strategies on all-cause mortality, SAEs, quality of life, lung injuries, myocardial infarction, stroke, and sepsis at maximum follow-up. This is due to low or very low-certainty evidence.PMID:37700687 | DOI:10.1002/14651858.CD012631.pub3

CROI 2023: Neuropsychiatric Complications in People With HIV
Top Antivir Med. 2023 Jun 30;31(4):543-555.ABSTRACTThe 2023 Conference on Retroviruses and Opportunistic Infections (CROI) featured new and impactful findings about neuropsychiatric complications in people with HIV and other infections. Reports included new evidence of (a) the importance of myeloid cells in the pathogenesis of HIV disease in the central nervous system, including as an HIV reservoir; (b) eukaryotic and prokaryotic viruses in cerebrospinal fluid during suppressive antiretroviral therapy; (c) the influence of sex on pathogenesis, including in novel neuropsychiatric biotypes identified by machine learning and ...

CROI 2023: Tuberculosis and Infectious Complications in Persons With HIV
Top Antivir Med. 2023 Jun 30;31(4):529-537.ABSTRACTNovel implementation strategies to increase uptake and adherence to tuberculosis (TB) preventive therapy hold promise for reducing TB incidence in persons with HIV in high-burden settings. In persons who develop drug-susceptible TB, progress to shorten TB treatment continues to be made with the introduction of new drugs and novel treatment strategies that could allow for treatment shortening to 2 months for most people. A global case series provided powerful evidence that mpox should be considered an HIV-related opportunistic infection given its severe manifestations and p...

CROI 2023: Metabolic and Other Complications of HIV Infection
Top Antivir Med. 2023 Jun 30;31(4):538-542.ABSTRACTComorbid conditions have major impacts on the health, quality of life, and survival of people with HIV, particularly as they age. The 2023 Conference on Retroviruses and Opportunistic Infections (CROI) featured excellent science related to specific comorbidities, such as cardiovascular disease (CVD), cancer, and obesity. Studies investigating factors that may contribute to CVD, such as mental health disorders, antiretroviral therapies, and activation of hormonal pathways, were featured prominently. Other studies sought to understand the epidemiology of non-AIDS-defining ca...

CROI 2023: Summary of Basic Science Research in HIV
Top Antivir Med. 2023 Jun 30;31(4):523-528.ABSTRACTThe 2023 Conference on Retroviruses and Opportunistic Infections (CROI) represented the first fully in-person conference since the severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) pandemic began. CROI continues as the premier conference in which delegates can appraise themselves of almost every facet of HIV/AIDS research as well as emerging and re-emerging pathogens such as SARS-CoV-2 and mpox. The return to an in-person format is particularly important for early-stage investigators, who were faced with challenges of advancing their independent research careers...

Impact of Primary Breast Surgery on Overall Survival of Patients With De Novo Metastatic Breast Cancer: A Systematic Review and Meta-Analysis
CONCLUSION: Surgery of the primary tumor in patients with de novo MBC does not prolong survival, except possibly in younger/premenopausal patients. Breast surgery should be offered within the context of well-designed clinical trials examining the issue.PMID:37700450 | DOI:10.1093/oncolo/oyad266
