Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy (CADASIL) – Still to be Considered in the Presence of Vascular Risk Factors
We report a 46-year-old male with a 9 and 3-month history of progressive unilateral lower limb weakness and dysarthria, respectively. He had a history of diabetes mellitus but no hypertension, hyperlipidemia, or smoking history. Both parents had a stroke at the age of 65 years. Neurological examination was significant for moderate dysarthria and reduced right upper limb dexterity. Asymptomatic Carotid Stenosis Is Associated With Circadian and Other Variability in Embolus Detection
Conclusions: Embolism associated with asymptomatic carotid stenosis shows circadian variation with highest rates 4–6 h before midday. This corresponds with peak circadian incidence of stroke and other vascular complications. Aggressive Medical Management for ICAD: A Comparison to the SAMMPRIS Trial (P1.174)
Conclusions:In a single center observational cohort study, we found that AMM in symptomatic ICAD yielded higher rates of recurrent stroke at 30 days when compared to the SAMMPRIS trial. Crossed Subcortical Aphasia (P1.048)
CONCLUSIONS: This patient is a rare case in which right-sided subcortical damage results in aphasia. The most comprehensive evidence on language lateralization comes from functional studies of healthy adults that differ in their degree of handedness. Urgent Best Medical Therapy May Obviate the Need for Urgent Surgery in Patients With Symptomatic Carotid Stenosis Clinical Sciences
Conclusions— CEA can be performed in the subacute period without significantly increasing the operative risk. The urgent best medical treatment was associated with significant reduction in the risk of early NR in CEA patients. It seems that urgent aggressive best medical treatment may obviate the need for urgent CEA.
