Identification of vulnerable carotid plaque with histologically validated CT-derived plaque maps
CONCLUSIONS: Carotid plaque components can be differentiated by CT using a ratio of plaque/contrast attenuation. CT-derived plaque map volumes of necrotic core help distinguished the most vulnerable plaques.ADVANCES IN KNOWLEDGE: CT-derived plaque maps based on plaque/contrast attenuation may provide new markers of carotid plaque vulnerability.PMID:37183910 | DOI:10.1259/bjr.20220982

Cost-effectiveness of carotid endarterectomy in symptomatic patients
CONCLUSION: Surgery may not now be clinically effective and cost-effective in those with moderate carotid stenosis. However, these results are uncertain because of the limited data on modern medical therapy and an RCT may be justified.PMID:36422995 | DOI:10.1093/bjs/znac386

Carotid Endarterectomy Following Intravenous Thrombolysis in the UK
CONCLUSION: The use of ivTT before CEA in stroke patients was not associated with an increased risk of 30 day stroke, but there was an increase in the risk of neck haematoma.PMID:34088616 | DOI:10.1016/j.ejvs.2021.03.033

Timing of procedural stroke and death in asymptomatic patients undergoing carotid endarterectomy: individual patient analysis from four RCTs.
CONCLUSION: At least half of the procedural strokes in this study were ischaemic and ipsilateral to the treated artery. Half of all procedural complications occurred on the day of surgery, but one-third after day 3 when many patients had been discharged. PMID: 32162310 [PubMed - as supplied by publisher]

Meta-analysis of the outcomes of treatment of internal carotid artery near occlusion.
CONCLUSION: These data suggest that BMT alone is not superior to CEA or CAS with respect to 30-day or 1-year stroke or death prevention in patients with ICANO. These patients do not appear to constitute a high-risk group for surgery, and consideration should made to including them in future RCTs of internal carotid artery interventions. Association between physical activity and sedentary behaviour on carotid atherosclerotic plaques: an epidemiological and histological study in 90 asymptomatic patients.
CONCLUSION: In this cross-sectional study of asymptomatic patients who underwent endarterectomy (i) higher reported PA, (ii) intensity of PA and (iii) lower reported SB were associated with lower prevalence of intraplaque haemorrhage. This could be a mechanism whereby PA protects against cerebrovascular disease (stroke) and death. PMID: 30842104 [PubMed - as supplied by publisher]

Meta-analysis of the procedural risks of carotid endarterectomy and carotid artery stenting over time.
CONCLUSIONS: Risks of procedural stroke/death following CEA appear to have decreased substantially. There was no evidence of a change in stroke/death rates following CAS. PMID: 29205297 [PubMed - as supplied by publisher]

Meta-analysis of the costs of carotid artery stenting and carotid endarterectomy.
CONCLUSION: Hospitalization and long-term costs of CAS and CEA appear similar. Economic considerations should not influence the choice of stenting or surgery in patients with carotid artery stenosis being considered for revascularization. PMID: 28783225 [PubMed - in process]
