CONCLUSIONS: Brain abscess or ischemic stroke must be expected in patients with history of OWRS who present with neurologic manifestations. Family members of such patients should be screened for OWRS with appropriate genetic testing. All the patients with OWRS, whether symptomatic or not, should be screened for pulmonary AVMs. If found, pulmonary AVM’s should be treated to prevent paradoxical embolism which can result in brain abscess or ischemic stroke, both of which can be potentially fatal. Intramedullary spinal cord abscess associated with right-to-left shunt via right superior vena cava draining into left atrium: A case report
We report the first case of ISCA associated with RSVC-LA RL shunt. Patient concerns: A 36-year-old man developed progressive paraparesis, dysuria, and spontaneous pain in the lumbar region and lower extremities. Spinal magnetic resonance imaging revealed an intramedullary lesion extended from Th12 to L2 with ring-shaped gadolinium enhancement. Cerebrospinal fluid (CSF) study exhibited a marked pleocytosis, and CSF culture grew Streptococcus intermedius. Artery of Percheron Occlusion in China: A Case Report and Chinese Literature Review
Conclusion: Our report suggests that unusual mood disorder and language disorder of aged patients might indicate the AOP occlusion, and cerebral imaging of magnetic resonance imaging (better with magnetic resonance angiography) should be performed to establish the diagnosis of AOP occlusion. The fast and accurate diagnosis of stroke because of AOP occlusion could best benefit the patients.
