Sociodemographic disparities in concomitant left atrial appendage occlusion during cardiac valve operations
by Ayesha P. Ng, Nikhil Chervu, Yas Sanaiha, Amulya Vadlakonda, Elsa Kronen, Peyman Benharash BackgroundSociodemographic disparities in atrial fibrillation (AF) management and thromboembolic prophylaxis have previously been reported, which may involve inequitable access to left atrial appendage occlusion (LAAO) during cardiac surgery. The present study aimed to evaluate the association of LAAO utilization with sex, race, and hospital region among patients with AF undergoing heart valve operations. MethodsAdults with AF undergoing valve replacement/repair in the 2012 –2019 National Inpatient Sample were identified and str...

Role of IL-17A in different stages of ischemic stroke
Int Immunopharmacol. 2023 Apr;117:109926. doi: 10.1016/j.intimp.2023.109926. Epub 2023 Mar 11.ABSTRACTInterleukin-17A (IL-17A) plays an important role in the progression of ischemic stroke. IL-17A mediates the endothelial inflammatory response, promotes water and sodium retention, and changes the electrophysiological structure of the atrium, accelerating the progression of ischemic stroke risk factors such as atherosclerotic plaques, hypertension, and atrial fibrillation. In the acute phase of ischemic stroke, IL-17A mediates neuronal injury through neutrophil chemotaxis to the site of injury, the induction of neuronal apo...

Left atrial appendage closure during mitral repair in patients without atrial fibrillation
Routine left atrial appendage closure during mitral repair in patients without atrial fibrillation (AF) is controversial. We aimed to compare the incidence of stroke after mitral repair in patients without recent AF according to left atrial appendage closure.

Closure of Left Atrial Appendage Has no Effect on Thromboembolic Rates after Mitral Valve Repair in Patients in Sinus Rhythm
This study examined the effects of closing the LAA in TIA/stroke after MV repair. Our database on patients who had MV repair for leaflet prolapse from 2000 through 2019 was reviewed. After excluding patients at higher risk of TIA/stroke, 1,050 patients in sinus rhythm were entered into the study: 781 with open LAA and 269 with surgically closed LAA.

The impact of prior cardiac surgery on patients undergoing surgical repair for acute type A aortic dissection
ConclusionsThese findings suggest that re-sternotomy can be safely performed with similar outcomes as first-time sternotomy. Central initiation of CPB after sternal reentry limits CPB time and may therefore represent a protective strategy that enhances outcomes for patients presenting with ATAAD and prior cardiac surgery.

Risk of Stroke and Major Bleeding with Vitamin-K Antagonist Use after Mitral Valve Repair
CONCLUSIONS: Our study suggests that VKA after MVr does not reduce risk of cerebral embolic events but is associated with increased risk of major bleeding events.PMID:36223805 | DOI:10.1016/j.athoracsur.2022.09.038

Long-term Outcomes of Concomitant Surgical Ablation for Atrial Fibrillation
CONCLUSIONS: Concomitant atrial fibrillation ablation during cardiac surgery is safe, does not increase the rate of perioperative complications, and confers the benefit of long-term survival after cardiac surgery in adults. Atrial fibrillation ablation also improved cardiac surgery patients' long-term survival compared with the matched general atrial fibrillation population.PMID:36216085 | DOI:10.1016/j.athoracsur.2022.09.036

Apixaban for Anticoagulation after Robotic Mitral Valve Repair
CONCLUSIONS: Anticoagulation with apixaban after minimally invasive robotic MVRep is safe and has similar rates of bleeding and thromboembolism compared to patients treated with warfarin.PMID:35973484 | DOI:10.1016/j.athoracsur.2022.07.045

Sex differences in acute type A aortic dissection: a systematic review and meta-analysis
CONCLUSIONS: Composite analysis indicates that early mortality does not differ between the sexes; however, late outcomes favour males. Differences in preoperative presentation and subsequent procedure selection between the sexes likely contribute to the disparity in late outcomes. Decision-making for surgical treatment of ATAAD should account for sex-specific risk factors.PMID:35687063 | DOI:10.23736/S0021-9509.22.12273-1
