The association between intraoperative fluid management and perioperative allogenic blood transfusion during adolescent idiopathic scoliosis surgery
CONCLUSION: These results indicate a statistical association between the increase in crystalloid intake and the risk of allogenic blood transfusion during surgical correction of adolescent idiopathic scoliosis. Controlled studies are needed to address the causative relation between intraoperative fluid intake and the risk of allogenic transfusion.PMID:37386846 | DOI:10.1111/pan.14722

Isoflurane Enhances Autophagy by Activating AMPK/ULK1, Inhibits NLRP3, and Reduces Cognitive Impairment After Cerebral Ischemia –Reperfusion Injury in Rats
AbstractCerebral ischemic stroke (CIS) has become the second leading cause of death worldwide, which is largely related to cerebral ischemia reperfusion injury (CIRI). Surgical intervention is a reliable treatment for CIS, which predictably causes cerebral reperfusion. Therefore, the choice of anesthetic drugs has important clinical significance. Isoflurane (ISO), one of the most used anesthetics, attenuates cognitive impairment and has brain protective effects. However, the role of isoflurane in regulating autophagy and its regulatory mechanism on inflammation in CIRI are still unclear. The middle cerebral artery occlusio...

Systemic lupus erythematosus associated with development of macrophage activation syndrome and disseminated aspergillosis
CONCLUSIONS: Four features of this case merit discussion, including the: 1) infrequent association of SLE with MAS; 2) short interval between SLE diagnosis and critical illness; 3) manifestation of fungal tracheobronchitis with airway obstruction; and 4) lack of response to antifungal treatment while receiving ECMO.PMID:37349668 | DOI:10.1007/s12630-023-02506-2

Association of cervical atherosclerosis on Doppler ultrasonography and postoperative delirium in patients undergoing spinal surgery: a single-center, retrospective, observational study
CONCLUSIONS: There was a significant association between POD and the prevalence of cervical atherosclerosis using the univariate logistic regression analysis. Furthermore, multivariate logistic regression analyses showed that older age and antiplatelet agent use were independently associated with POD.PMID:37326838 | DOI:10.1007/s00586-023-07823-0

The Effect of Intraoperative Hypotension on Postoperative Renal Function
AbstractPurpose of ReviewThis review summarizes the most recent literature on the association between intraoperative hypotension (IOH) and the occurrence of postoperative acute kidney injury (AKI). It provides recommendations for the management of intraoperative blood pressure to reduce the incidence of postoperative AKI. Fluid management strategies, administration of vasopressor medications, and other methods for reducing the incidence of AKI are also briefly discussed.Recent FindingsRecent retrospective studies have demonstrated a solid association of IOH with postoperative AKI. IOH is associated not only with AKI but al...

“The peripheral perfusion index discriminates haemodynamic responses to induction of general anaesthesia”
AbstractInduction of general anaesthesia is often accompanied by hypotension. Standard haemodynamic monitoring during anaesthesia relies on intermittent blood pressure and heart rate. Continuous monitoring systemic blood pressure requires invasive or advanced modalities creating a barrier for obtaining important information of the circulation. The Peripheral Perfusion Index (PPI) is obtained non-invasively and continuously by standard photoplethysmography. We hypothesized that different patterns of changes in systemic haemodynamics during induction of general anaesthesia would be reflected in the PPI. Continuous values of ...

Driving the wrong way on a one-way street. Reversal of coronary flow in aortic regurgitation
Aortic regurgitation (AR) represents the quintessential left ventricular volume overload state. In contrast to mitral regurgitation, where the left ventricle (LV) ejects blood into both the relatively low pressure left atrium and higher-pressure aorta, in AR, the entirety of LV stroke volume is ejected into the relatively higher pressure aorta. The severity of AR is obviously related to the regurgitant volume ejected into the LV during diastole. When severe, the forward stroke volume available for tissue perfusion represents a minority of the cardiac output.

General anesthesia or conscious sedation for thrombectomy in stroke patients: an updated systematic review and meta-analysis
CONCLUSIONS: In this updated systematic review and meta-analysis, the choice of anesthetic strategy for endovascular treatment of stroke patients did not significantly impact functional outcome as measured with the mRS at three months. Patients managed with general anesthesia may have more frequent successful reperfusion.TRIAL REGISTRATION: PROSPERO (CRD42022319368); registered 19 April 2022.PMID:37268801 | DOI:10.1007/s12630-023-02481-8
