Cerebral embolic protection during transcatheter heart interventions
EuroIntervention. 2023 Sep 18;19(7):549-570. doi: 10.4244/EIJ-D-23-00166.ABSTRACTStroke remains a devastating complication of transcatheter aortic valve replacement (TAVR), with the incidence of clinically apparent stroke seemingly fixed at around 3% despite TAVR's significant evolution during the past decade. Embolic showers of debris (calcium, atheroma, valve material, foreign material) are captured in the majority of patients who have TAVR using a filter-based cerebral embolic protection device (CEPD). Additionally, in systematic brain imaging studies, the majority of patients receiving TAVR exhibit new cerebral lesions...

Polyuria, Polydipsia and Possible Diabetes Insipidus?
Discussion The body is smart. It has mechanisms for maintaining balances within the body in a closely controlled manner but allows for a variety of states. This is very true for fluid balance in the body which is highly controlled between almost all of the major body organ systems. When fluid is low, the sensor sends signals for us to drink, and conserves fluid until we can. When the fluid is high, the kidneys excrete the excess and sends signals not to drink. Usually it works very well. While there are many pathological states that can cause polyuria and/or polydipsia, the most common reason is excessive fluid intake beca...

What Is New in Low Gradient Aortic Stenosis: Surgery, TAVR, or Medical Therapy?
AbstractBackgroundA significant proportion of patients with aortic stenosis (AS) have discordance in severity by mean gradient/peak velocity and aortic valve area. Low gradient aortic stenosis (LG-AS) is defined when the aortic valve area is<  1 cm2 consistent with severe AS and mean aortic gradient is<  40 mmHg consistent with non-severe AS. LG-AS represents a diagnostic and therapeutic challenge.Purpose of ReviewTo summarize the different categories, diagnosis, management, and prognosis of LG-AS. LG-AS is classified as classical (ejection fraction (EF)<  50%, indexed stroke volume (SVi) <  35 ml/...

Investigation of Computed-Tomography Based Predictors of Acute Stroke Related to Transcatheter Aortic Valve Replacement: Aortic Wall Plaque Thickness Might be a Predictive Parameter of Stroke.
CONCLUSION: Aortic wall plaque thickness measured by contrast-enhanced CT might be a predictive parameter of strokes that occur within 48 hours after TAVR. PMID: 32005786 [PubMed - in process]

Does Mitral Valve Calcium In Patients Undergoing Mitral Valve Replacement Portend Worse Survival?
ConclusionsConservative approach to treat MAC achieves satisfactory results. Patients with MAC have significant comorbidities contributing to a worse survival though MAC in itself is not a risk factor for mortality.

Report: Extra insights, but no clear-cut answers from TAVR embolic protection studies
Data from two new studies has provided extra insights, but no clear-cut solution for predicting stroke and which patients should receive embolic protection devices during transcatheter aortic valve replacement procedures, according to a new Medscape report. Results from a meta-analysis of 1,285 patients indicated that the use of embolic protection devices during TAVR procedures did not reduce mortality, but was associated with a lowered rate of stroke at 30 days, according to the report. The decline was mainly driven by registry data and was not confirmed by randomized controlled trials, Medscape specified. Stroke was repo...

Paradoxical aortic stenosis: A systematic review
Publication date: Available online 31 March 2017 Source:Revista Portuguesa de Cardiologia (English Edition) Author(s): Rita Cavaca, Rogério Teixeira, Maria João Vieira, Lino Gonçalves Aortic stenosis (AS) is a complex systemic valvular and vascular disease with a high prevalence in developed countries. The new entity “paradoxical low-flow, low-gradient aortic stenosis” refers to cases in which patients have severe AS based on assessment of aortic valve area (AVA) (≤1 cm2) or indexed AVA (≤0.6 cm2/m2), but paradoxically have a low mean transvalvular gradient (<40 mmHg) and a low stroke volume index (≤35...

Avoiding coronary obstruction after transcatheter aortic valve replacement: Is it the skirt or what's inside that counts?
In this issue of the Journal, Chu and colleagues1 posit that coronary obstruction in patients at high risk undergoing transcatheter aortic valve replacement (TAVR) may be mitigated by the unique engagement mechanism and protective skirt of the ACURATE TA device (Symetis Inc, Ecublens, Switzerland). Initial obstacles with TAVR, including stroke, vascular complications, and paravalvular leak (PVL) have seen marked improvements through case selection and device design; however, the rare and lethal complication of coronary artery obstruction remains vexing, particularly in patients at high risk with small aortic diameters, low...

Feasibility and safety of transfemoral implantation of Edwards SAPIEN XT prosthesis without balloon valvuloplasty in severe stenosis of native aortic valve
ConclusionsDirect implantation of Edwards SAPIEN XT without prior BV in selected cases is feasible and safe. The number of patients in whom this technique would be applicable, and their impact on reducing complications has to be determined. © 2013 Wiley Periodicals, Inc.

Aortic valve calcium score as a predictor for outcome after TAVI using the CoreValve revalving system
Abstract: Background: TAVI is a novel treatment option for patients at too high risk for surgery. Risk scores for surgical valve replacement failed to accurately predict outcomes after TAVI and alternative risk parameters are lacking so far. Objective: We evaluated the CT-derived aortic valve calcification score as a predictor for outcome during and after TAVI.Methods: Transfemoral TAVI using the CoreValve device was performed in 68 patients, in whom the aortic valve calcium score was determined from preprocedural 64-sclice ECG gated CT-scans.Results: 30-day MACE rate (death, stroke, MI) was 10.3%, 1-year mortality was 11.8...
