Outcome after acute ischemic stroke treatment during Covid-19 outbreak in South-East Tuscany
CONCLUSION: During the lockdown period due to Covid-19 pandemic, patients with acute ischemic stroke had a worse prognosis. These findings suggest the need to improve the health system organization to guarantee an appropriate treatment during the pandemic, including the patients that are not affected by Covid-19.PMID:34579640 | DOI:10.2174/1871529X21666210927094058

Mechanical thrombectomy in acute ischemic stroke due to large vessel occlusion in the anterior circulation and low baseline National Institute of Health Stroke Scale score: a multicenter retrospective matched analysis
ConclusionsOur experience is in favor of a potential benefit of MT also in patients with LVO and a NIHSS score  ≤ 5 at the time of groin puncture. Nonetheless, this issue waits for a clear-cut recommendation in a dedicated clinical trial.

Brivaracetam as add-on treatment in patients with post-stroke epilepsy: real-world data from the BRIVAracetam add-on First Italian netwoRk Study (BRIVAFIRST)
Stroke is a leading disease worldwide, with an estimated incidence of more than 15 million cases annually [1]. Post-stroke epilepsy (PSE), which is defined as the occurrence of one or more unprovoked epileptic seizures at least one week after the stroke, develops in at least 4-6% of the stroke population [2]. Cerebrovascular disease is one of the most common causes of acquired epilepsy and accounts for about 10-15% of all newly diagnosed epilepsy cases [2]. Although PSE has overall a good prognosis and patients are generally responsive to pharmacological treatment, approximately 20% of the patients are pharmaco-resistant [3,4].

Sex differences in outcome after thrombectomy for acute ischemic stroke. A propensity score-matched study
CONCLUSIONS: Subject to the limitations of a non-randomized comparison, women with stroke due to LVO treated with mechanical thrombectomy had a better chance to achieve complete recanalization, and 3-month functional independence than men. The results could be driven by women who underwent combined treatment.PMID:35647312 | PMC:PMC9134778 | DOI:10.1177/23969873221091648

Bringing door-to-needle times within the European benchmarks results in better  stroke patients outcomes in a spoke hospital from the Apulian Region
AbstractIntroductionDoor-to-needle time (DNT) is a key factor in acute stroke treatment success. We retrospectively analysed the effects of a new protocol aimed at reducing treatment delays in our single-centre observational series over a 1-year period (from October 1st 2021 to September 30th 2022).MethodsThe time frame was divided into two semesters as a new protocol was started at the beginning of the second semester to ensure a rapid evaluation, imaging, and intravenous thrombolysis in all stroke patients attending our spoke-hospital serving 200,000 inhabitants. Logistics and outcome measures were obtained for each pati...

Stroke with large vessel occlusion in the posterior circulation: IV thrombolysis plus thrombectomy versus IV thrombolysis alone
AbstractEfficacy and safety of mechanical thrombectomy (MT) for stroke with posterior circulation large vessel occlusion (LVO) is still under debate. We aimed to compare the outcomes of stroke patients with posterior circulation LVO treated with intravenous thrombolysis (IVT) (<  4.5 h after symptom onset) plus MT <  6 h after symptom onset with those treated with IVT alone (<  4.5 h after symptom onset). Patients enrolled in the Italian Registry of Endovascular Treatment in Acute Stroke (IRETAS) and in the Italian centers included in the SITS-ISTR were analysed. We identified 409 IRETAS patients treate...

Incidence of orolingual angioedema after intravenous thrombolysis for stroke
DiscussionThe incidence of orolingual angioedema was very low in our series. Although OA is usually mild, anaphylactoid reactions may rarely occur, because of the variable degree of activation of the complement system and kinin cascade caused by alteplase. In such instances, admission to neurointensive care may be required. Specific bradykinin antagonists or drugs that target the kallikrein-kinin system are beginning to be used in the more severe cases. Thus, doctors and nurses caring for acute stroke patients need to be able to recognize and treat this complication.

Efficacy of Robotic-Assisted Gait Training in sub-acute stroke patients: An Italian bi-centre study
Introduction: Over the last years, the introduction of robotic technologies in gait rehabilitation of stroke patients has had a greatest interest. Some studies have been conducted to assess the effects of robot-assisted training compared to gait conventional rehabilitation in sub-acute stroke patients. The main results were obtained in studies that used robotic exoskeletons or treadmill training with partial body weight support and only few papers used end-effector device [1]. To date, there are very few studies that have used gait analysis to show improvements in gait parameters after gait rehabilitation in sub-acute stroke patients [2].

Initial experience with the novel EmboTrap II clot-retrieving device for the treatment of ischaemic stroke.
CONCLUSION: In our experience, mechanical thrombectomy with EmboTrap II is safe and effective. Reperfusion rate was comparable to that obtained with other stent retrievers. PMID: 30714502 [PubMed - as supplied by publisher]

Stroke-Like Presentation of Paraneoplastic Cerebellar Degeneration: a Single-Center Experience and Review of the Literature
We report herein incidence and clinical features of hyperacute onset PCD, a vertebrobasilar stroke mimic. We performed a retrospective analysis of all suspected PCD cases referred to the Udine University Hospital between 2009 and 2017. Our center provides the only neuroimmunology laboratory for three provinces of the Friuli-Venezia Giulia region, Italy (983,190 people as of January 1, 2017). Inclusion criteria were (1) abrupt onset of neurological symptoms; (2) initial consideration of a vascular etiology; (3) final diagnosis of “definite PCD.” We also carried out a systematic review of the literature in order to ident...

Failure of Therapeutic Anticoagulation in COVID-19 Patients With Acute Ischemic Stroke. A Retrospective Multicenter Study
ConclusionsThe occurrence of an anticoagulation failure seems to be increased in the setting of COVID-19 infection, with worse clinical outcomes if compared to non-COVID-19 related ischemic strokes. We discuss the diagnostic and therapeutic implications of such evidence, suggesting that some arterial thrombotic complications might be either resistant to or independent of the anticoagulation effect.
