Discussion During fetal development, the heart primum and secundum septa grow and overlap leaving a small but important channel between the two atria. The foramen ovale is a flap valve moving blood from the right atrium into the left atrium directly and bypassing the high pressure pulmonary system. After birth and breathing air, the neonate’s lungs open up and the pulmonary vascular resistance decreases. Transapical Aortic Valve Replacement for Severe Aortic Stenosis: Results From the Nonrandomized Continued Access Cohort of the PARTNER Trial.
CONCLUSIONS: Among the 975 patients in the NRCA-TA cohort, rates of major outcomes including death and stroke compared favorably with outcomes of PMA-TA and SAVR patients enrolled in the PARTNER trial. This trend toward improved outcomes may be attributed to improved patient selection, individual centers surmounting the procedural learning curve, and refinements in surgical technique. ConclusionsParasternal minimally ‐invasive aortic valve replacement is a feasible technique associated with a slow learning curve but the potential to achieve improved outcomes. Considering the transcatheter alternatives, the relative risk reduction may be worth investigating in future trials.

Initial experience with minimally invasive cardiac operations
CONCLUSIONS: Our initial results with minimally invasive operations demonstrated that it was safe and with good clinical results. Patient satisfaction is quite high. After the learning phase has been transversed, minimally invasive operations may be an excellent alternative for many patients with congenital and acquired diseases.

Conclusions: Implantation of the Perceval S sutureless bioprosthesis through the right chest can be performed safely with progressive decreases in cardiopulmonary bypass, cross-clamp, and procedural times. CUSUM analysis shows a small learning curve with evidence of plateau after the first 10 cases.
