Fourth mitral valve replacement in a 15-year span in a woman of childbearing age
Publication date: Available online 13 June 2018Source: Journal of Cardiology CasesAuthor(s): Laura Leticia Rodríguez-Chávez, Grecia Iveth Maryelis Raymundo-Martínez, Rodrigo Gopar-NietoAbstractThere is no report of four valve replacement surgeries in a woman during 15-year follow-up. A 23-year-old female was diagnosed at 7 years with rheumatic fever. She developed severe mitral regurgitation and at the same age a prosthetic valve was placed. In the next 15 years her mitral valve was changed 3 times because of clinical, echocardiographic, and histopathologic evidence of thrombosis. Her coagulation tests all showed infrat...

Long-standing persistent effects of atrial volume reduction combined with pulmonary vein isolation
Publication date: Available online 18 June 2018Source: Journal of Cardiology CasesAuthor(s): Masami Takagaki, Naoko Ikeda, Hiroki Yamaguchi, Shinichi Mitsuyama, Tasuku Kadowaki, Tatsuya NakaoAbstractAlthough surgical left atrial (LA) volume reduction combined with mitral valve surgery and/or surgical ablation for atrial fibrillation has been reported to be effective, its long-term outcomes in the absence of mitral procedure are not well established. A 74-year-old man with two previous sternotomies—the first for pericardiectomy due to constrictive pericarditis and the second for mitral valve replacement with mechanical va...

Resuscitation Education Science: Educational Strategies to Improve Outcomes From Cardiac Arrest: A Scientific Statement From the American Heart Association.
Education Science and Programs Committee, Council on Cardiopulmonary, Critical Care, Perioperative and Resuscitation; Council on Cardiovascular and Stroke Nursing; and Council on Quality of Care and Outcomes Research Abstract The formula for survival in resuscitation describes educational efficiency and local implementation as key determinants in survival after cardiac arrest. Current educational offerings in the form of standardized online and face-to-face courses are falling short, with providers demonstrating a decay of skills over time. This translates to suboptimal clinical care and poor survival outcomes fr...

Medical Nutrition Education, Training, and Competencies to Advance Guideline-Based Diet Counseling by Physicians: A Science Advisory From the American Heart Association.
lar and Stroke Nursing; Council on Cardiovascular Radiology and Intervention; and Stroke Council Abstract Growing scientific evidence of the benefits of heart-healthy dietary patterns and of the massive public health and economic burdens attributed to obesity and poor diet quality have triggered national calls to increase diet counseling in outpatients with atherosclerotic cardiovascular disease or risk factors. However, despite evidence that physicians are willing to undertake this task and are viewed as credible sources of diet information, they engage patients in diet counseling at less than desirable rates and...

Comparing learning curves of two established “single-shot” devices for ablation of atrial fibrillation
ConclusionsWith either of the systems, no significant differences in the effect of the learning curve on the occurrence of adverse events were observed. However, the PVAC array seemed to have a steeper learning curve for procedure, as well as fluoroscopy time.

Minimally Invasive Mitral Valve Repair Through Right Minithoracotomy - 11-Year Single Institute Experience.
CONCLUSIONS: MIMVR can be performed safely with low levels of mortality and morbidity, and provides sufficient repair durability. A learning curve exists in terms of repair durability, especially for anterior mitral leaflet repair. PMID: 29657253 [PubMed - as supplied by publisher]

The Role of the Left Atrial Appendage in Stroke and Arrhythmia Provocation
AbstractPurpose of the reviewThe review will provide an overview of the potential use of left atrial appendage (LAA) exclusion in patients with atrial fibrillation (AF), highlighting the benefits and risks involved with LAA exclusion.Recent findingsLAA ligation leads to electrical isolation of the LAA and a decrease in LA mass which is associated with a decrease in AF burden and occasional termination of AF in patients with persistent AF. This potential new indication will further expand the use of LAA exclusion and necessitate planning of combining LAA exclusion with catheter ablation for both stroke prevention and antiar...

Learning and Teaching Electrocardiography in the 21st Century: A Neglected Art
It is now over a century since the introduction of electrocardiography (ECG) by Willelm Einthoven in 1902 [1,2]. The ECG is probably one of the most commonly used test in clinical practice, and, unlike many other clinical tests, the ECG results are available immediately. This is important for an emergency condition where an immediate decision should be made such as in the case of acute myocardial infarction or stroke. Professor Hein Wellens stated that, “Worldwide, approximately 3 million ECGs are made daily.

First results of the Brazilian Registry of Percutaneous Left Atrial Appendage Closure
Conclusions: In this multicenter, real world registry, that included patients with NVAF and high thromboembol ic and bleeding risks, LAAC effectively prevented stroke and bleeding when compared to the expected rates based on CHA2DS2VASc and HASBLED scores for this population. Complications rate of the procedure was acceptable considering the beginning of the learning curve of most of the involved operators.Resumo Fundamento: A oclus ão percutânea do apêndice atrial esquerdo (OAAE) é uma alternativa eficaz à anticoagulação oral (ACO) para a prevenção de acidente vascular cerebral (AVC) em pacientes com fibrilação...
