Characteristics of Randomized Trials Focusing on Stroke due to Intracerebral Hemorrhage Clinical Sciences
Background and Purpose—The absence of treatments for intracerebral hemorrhage with significant consistent benefit in randomized controlled trials (RCTs) could be because of lack of treatment efficacy or the design of RCTs.Methods—We searched the Cochrane Stroke Group Trials Register in December 2015 for completed and published RCTs reporting clinical outcome in adults with intracerebral hemorrhage. We collected data on publication year and language, study characteristics, and effect size. We regarded RCTs to be at lower risk of bias if they performed ≥2 of describing randomization, using blinding, or specifying the p...

Organised inpatient (stroke unit) care for stroke: network meta-analysis.
CONCLUSIONS: We found moderate-quality evidence that stroke patients who receive organised inpatient (stroke unit) care are more likely to be alive, independent, and living at home one year after the stroke. The apparent benefits were independent of patient age, sex, initial stroke severity, or stroke type, and were most obvious in units based in a discrete stroke ward. We observed no systematic increase in the length of inpatient stay, but these findings had considerable uncertainty. PMID: 32324916 [PubMed - as supplied by publisher]

World Stroke Organization Global Stroke Services Guidelines and Action Plan
Every two seconds, someone across the globe suffers a symptomatic stroke. ‘Silent’ cerebrovascular disease insidiously contributes to worldwide disability by causing cognitive impairment in the elderly. The risk of cerebrovascular disease is disproportionately higher in low to middle income countries where there may be barriers to stroke care. The last two decades have seen a major transformation in the stroke field with the emergence of evidence‐based approaches to stroke prevention, acute stroke management, and stroke recovery. The current challenge lies in implementing these interventions, particularly in regions ...

Past, present, and future of stroke in middle‐income countries: the Brazilian experience
ConclusionsThe evolution of stroke care in Brazil over the last decade is a pathway that exemplifies the challenges that middle‐income countries have to face in order to improve stroke prevention, treatment and rehabilitation. The reported Brazilian experience can be extrapolated to understand the past, present, and future of stroke care in middle‐income countries.

Risk Adjustment of Ischemic Stroke Outcomes for Comparing Hospital Performance: A Statement for Healthcare Professionals From the American Heart Association/American Stroke Association AHA/ASA Scientific Statement
Conclusions— There are a number of important methodological challenges in undertaking risk-adjusted outcome comparisons to assess the quality of stroke care in different hospitals. It is important for stakeholders to recognize these challenges and for there to be a concerted approach to improving the methods for quality assessment and improvement.

Clinical diagnostic tools for screening of perioperative stroke in general surgery: a systematic review
Perioperative stroke is a devastating complication that carries high mortality and functional disability. Unfortunately, residual anaesthesia and analgesia may obscure important warning signs and may lead to a delay in the assessment and treatment of major stroke after surgery. The purpose of this review is to examine the utility of existing stroke scales, for the recognition of perioperative stroke in the general surgical population. A total of 21 stroke scales have been described in the literature. Diagnostic performance was reported in 17 scales. The majority of the stroke scales were designed to evaluate current neurol...

Race/Ethnic Disparities for In-Hospital Mortality and Disability at Discharge after Acute Ischemic Stroke: Florida Puerto Rico Collaboration to Reduce Stroke Disparities (P5.287)
Conclusions:In our large Florida Stroke Registry, we observed lower in-hospital mortality, but greater disability at discharge after stroke among blacks in comparison to whites. This paradox needs further exploration to design and implement interventions to improve stroke outcome and reduce disparities in post-stroke disability.Disclosure: Dr. Simonetto has nothing to disclose. Plasma Angiopoietin-1 Is Lower After Ischemic Stroke and Associated With Major Disability But Not Stroke Incidence Clinical Sciences
Conclusions— Plasma Angpt1 concentrations are low after ischemic stroke particularly in patients with poor stroke outcomes at 3 months. Interventions effective at upregulating Angpt1 could potentially improve stroke outcomes.

Part II: predictors of depression after stroke and impact of depression on stroke outcome: an updated systematic review of observational studies
ConclusionsEarly identification of patients at high risk of depression after stroke, those with a history of depression and physical disability after stroke, would enable the early implementation of effective management and prevention strategies for depression. The reciprocal relationship between depression and physical disability highlights the need for interventions that reduce disability after stroke, which may in turn improve mood and overall recovery for an increasingly large number of stroke survivors.

Stroke Is the Leading Cause of Death in Rural Gadchiroli, India: A Prospective Community-Based Study Clinical Sciences
Conclusions— Stroke is the leading cause of death and accounted for 1 in 7 deaths in this rural community in Gadchiroli. There was high early mortality, and the mortality rate because of stroke was higher than that reported from previous studies from India. Stroke is emerging as a public health priority in rural India.

Factors for short-term outcomes in patients with a minor stroke: results from China National Stroke Registry
In this study, we aimed to identify which factors were associated with adverse outcomes of a minor stroke. Methods: The China National Stroke Registry (CNSR) is a nationwide prospective registry for patients presented to hospitals with acute cerebrovascular events between September 2007 and August 2008. The 3-month follow-up was completed in 4669 patients with a minor stroke defined as the initial neurological severity lower than 4 in the National Institutes of Health Stroke Scale (NIHSS). Multivariate model was used to determine the association between risk factors and clinical outcomes. Results: Of 4669 patients with a m...
