Combination of Alprazolam and Bailemian Capsule Improves the Sleep Quality in Patients With Post-Stroke Insomnia: A Retrospective Study
Insomnia is often ignored in the diagnosis and treatment of patients after stroke. The present study aimed to evaluate the efficacy of alprazolam (ALP) combined with Bailemian capsule (BC, a traditional Chinese patent medicine) in the treatment of post-stroke insomnia (PSI). A total of 231 stroke patients involved in this retrospective study were treated by ALP, BC, or ALP + BC for 3 weeks. The quality of sleep was evaluated by the Pittsburgh sleep quality index (PSQI) and polysomnography (PSG), while self-care ability was monitored by the modified Rankin Scale (mRS) before and after treatment. Compared to the baseline, th...

The Outcome of Status Epilepticus and Long-Term Follow-Up
Conclusion: This study highlights the importance of regular care and patient follow-up. Introduction Status epilepticus (SE) is a condition and most extreme form of epilepsy (1), which leads to abnormal and prolonged seizure (at least 5 min). In case SE persists over 30 min, it may have severe long-term consequences (2). Referring to the new classification scheme of SE, there are two operational dimensions of the definition: time point 1 (T1) is associated with abnormally prolonged seizure, when therapy should be initiated, while time point 2 (T2) is related to the time of on-going seizure activity involving a risk...

Probable Nootropic-induced Psychiatric Adverse Effects: A Series of Four Cases
Conclusion Healthcare providers in general, and specifically those in the mental health and substance abuse fields, should keep in mind that nootropic use is an under recognized and evolving problem. Nootropic use should be considered in cases where there are sudden or unexplained exacerbations of psychiatric symptoms in patients who have been stable and medication adherent. It is also important to remember that most nootropics are not detected on standard drug toxicology screening tests. We have very little clinical information on how nootropics may interact with psychotropics (or other medications) and potentially cause ...

Clinical Reasoning: A young woman with respiratory failure, hearing loss, and paraplegia
A 35-year-old woman with bipolar disorder presented to the emergency room (ER) obtunded with hypercapnic respiratory failure. Neurology was consulted because the patient had acute hearing loss and paraparesis. She was last seen normal the prior night by her ex-husband. Paramedics found her on the couch, obtunded, bradypneic, and hypoxic to 82% SpO2, blood pressure 116/79 mm Hg, heart rate 98 beats per minute, and normothermic. She awakened after 0.4 mg of naloxone administration. Home medications included oxycodone/acetaminophen for chronic pain and venlafaxine, alprazolam, and lamotrigine for bipolar disorder. O...

Comprehensive analysis of the diagnostic and therapeutic value of the hypoxia-related gene PLAUR in the progression of atherosclerosis
In this study, Plasminogen activator, urokinase receptor (PLAUR) was identified as an effective diagnostic marker for AS lesion progression by combining WGCNA and random forest algorithm. We validated the stability of the diagnostic value on multiple external datasets including humans and mice. We identified a significant correlation between PLAUR expression and lesion progression. We mined multiple single cell-RNA sequencing (sc-RNA seq) data to nominate macrophage as the key cell cluster for PLAUR mediated lesion progression. We combined cross-validation results from multiple databases to predict that HCG17-hsa-miR-424-5...
