Linking Australian Stroke Clinical Registry data with Australian government Medicare and medication dispensing claims data and the potential for bias
Aust N Z J Public Health. 2021 Apr 5. doi: 10.1111/1753-6405.13079. Online ahead of print.ABSTRACTOBJECTIVE: We aim to report the accuracy of linking data from a non-government-held clinical quality registry to national claims data and identify associated sources of systematic bias.METHODS: Patients with stroke or transient ischaemic attack admitted to hospitals participating in the Australian Stroke Clinical Registry (AuSCR) were linked with Medicare and medication dispensings through the Australian Medicare enrolment file (MEF). The proportion of registrants in the datasets was calculated and factors associated with a no...

Exploring facilitators and barriers to long ‐term behavior change following health–wellness coaching for stroke prevention: A qualitative study conducted in Auckland, New Zealand
ConclusionsEnhancing awareness of personal risk and the impact of potential disease are facilitators of long-term behavior change and should be incorporated into coaching conversations. This supports the process of “waking up” to health needs and the possibility of change, which are important precursors to long-term change. Health coaching should be responsive to individual need, with emotional well-being, happiness, and life satisfaction being addressed alongside physical health. The opportunity to develo p skills to support decision-making and self-management should be included in coaching initiatives, to enhance sel...

Economic evaluation of the Very Early Rehabilitation in SpEech (VERSE) intervention
CONCLUSION: There was limited evidence that additional intensively delivered aphasia therapy within the context of usual acute care provided was worthwhile in terms of costs for the outcomes gained.PMID:37415422 | DOI:10.1080/10749357.2023.2229039

Tenecteplase versus alteplase in acute ischemic stroke: systematic review and meta-analysis
AbstractTenecteplase is a product of genetic modification of recombinant tissue plasminogen activator with superior pharmacodynamic and pharmacokinetic properties. This meta-analysis was to determine whether intravenous thrombolysis with tenecteplase in patients with acute ischemic stroke has better efficacy and safety outcomes than with intravenous alteplase. PubMed, Cochrane Central Register of Controlled Trials, WHO International clinical trials registry platform (ICTRP), Australian New Zealand Clinical Trials Registry (ANZCTR), EU Clinical Trials Register (EU-CTR) and ClinicalTrials.gov were searched for trials compari...

Predictive values of pulse pressure variation and stroke volume variation for fluid responsiveness in patients with pneumoperitoneum
AbstractAnimal studies suggest that dynamic predictors remain useful in patients with pneumoperitoneum, but human data is conflicting. Our aim was to determine predictive values of pulse pressure variation (PPV) and stroke volume variation (SVV) in patients with pneumoperitoneum using LiDCORapid ™ haemodynamic monitor. Standardised fluid challenges of colloid were administered to patients undergoing laparoscopic procedures, one fluid challenge per patient. Intra-abdominal pressure was automatically held at 12  mmHg. Fluid responsiveness was defined as an increase in nominal stroke index (nSI) ≥ 10%. Linear regress...

Anticoagulant-related intracranial haemorrhage: time to anticoagulant reversal improving but still slower than thrombolysis for ischaemic stroke.
CONCLUSION: Door-to-any-reversal-agent and scan-to-PCC times have improved over time, but they remain significantly longer than IS thrombolysis times. Monitoring of reversal is inadequate, particularly for WRICH receiving PCC. PMID: 33582709 [PubMed - in process]
