Atrial fibrillation (AF) pilot screening programme in primary care in Ireland: an implementation study protocol
This study aims to investigate the acceptability, feasibility and impact of AF screening in primary care using a handheld mobile ECG device. Methods and analysis General practitioners (GPs) and practice nurses in the South of Ireland will opportunistically screen patients aged ≥65 years for AF at routine consultation using a handheld one-lead ECG device, KardiaMobile. Characteristics and management of patients with stroke and major hemorrhagic episodes with atrial fibrillation under vitamin K antagonist therapy. EVENTHO study
CONCLUSIONS: In the sample studied, half of the AF patients who suffered stroke/SE or major hemorrhagic episode had inadequate TTR and, despite this, after hospital discharge, they restarted treatment with VKA. These results highlight the need to evaluate safer and effective therapeutic alternatives in AF patients with poor TTR control after suffering a stroke/SE or major hemorrhagic episode.PMID:35120766 | DOI:10.1016/j.medcli.2021.12.004

Peripheral Arterial Disease, Stroke, and Mortality in Patients with Atrial Fibrillation
I read the paper by Vitalis et al1 with great interest. The authors assessed the impact of peripheral arterial disease on mortality and stroke in patients with atrial fibrillation. The adjusted hazard ratio (HR) (95% confidence interval [CI]) of peripheral arterial disease in patients with atrial fibrillation for all-cause mortality was 1.34 (1.06-1.70). In addition, the adjusted HR (95% CI) of peripheral arterial disease in patients with nonanticoagulated atrial fibrillation for ischemic stroke was 3.37 (1.25-9.09).
