Authors: Caixàs A, Villaró M, Arraiza C, Montalvá JC, Lecube A, Fernández-García JM, Corio R, Bellido D, Llisterri JL, Tinahones FJ Abstract The high prevalence of obesity in our environment, a chronic disease of complex management and responsible for multiple comorbidities, requires the implementation of coordination strategies in clinical care between primary care and specialist hospital units. In a cross-sectional care model, primary care physicians guide all therapeutic management related to obesity. Together with them, specialists in endocrinology and nutrition and other heal...

Incidence of malignancy in a Spanish cohort of patients infected by the human immunodeficiency virus.
The objective of this research was to estimate the incidence of malignancies in HIV patients and their surveillance. Age and sex-adjusted incidence was compared to the incidence calculated by the Spanish Cancer Registry network (REDECAN) in 2015. RESULTS: We included 2,773 patients (41,238 patients-year). Two hundred and eleven malignancies were diagnosed in 182 patients. Non-AIDS defining cancers accounted for 78.2% of the malignancies. The global incidence of cancer was 485 cases per 100,000 person-years. Twenty-year mortality rate was 31.2% in patients with cancer and 7.8% in patients without cancer. In men, adjuste...

Number of paraaortic lymph node dissections as a prognostic factor in locally advanced cervical cancer.
Authors: Nicolás I, Gilabert-Estellés J, Gilabert-Aguilar J, Fusté P, Aghababyan K, Pahisa J, Gil-Ibañez B, Díaz-Feijoo B, Carmona F, Calpe FJ, Saco A, Ordi J, Torne A Abstract BACKGROUND: Lymph node (LN) metastases are the most important prognostic factor in locally advanced cervical cancer. Paraaortic lymphadenectomy is the only method able to confirm the presence of metastasis and thereby help to determine the most adequate treatment approach. There is no standard regarding the minimal number of LNs that should be removed in paraaortic lymphadenectomy. Women with undiagnosed po...

New antibiotics for the treatment of infections by multidrug-resistant microorganisms.
Authors: Escolà-Vergé L, Los-Arcos I, Almirante B Abstract One of the current priorities of the World Health Organization is multidrug-resistant bacteria, because they are a global problem due to their rapid spread and the difficulty of their treatment. In addition, they are associated with high morbidity, mortality and high economic costs. There are multidrug-resistant bacteria, both Gram-positive and Gram-negative, including Pseudomonas aeruginosa and Acinetobacter baumannii resistant to carbapenems, enterobacteria producing carbapenemases, Staphylococcus aureus resistant to methicillin and/or with ...

Clinical applications of pulmonary ultrasound.
Authors: Wangüemert Pérez AL Abstract Pulmonary ultrasound is becoming very important for the diagnosis and monitoring of respiratory diseases in different areas, such as emergency departments, outpatient clinics, inpatient areas, etc. This review attempts to encompass most of the applications and utilities of thoracic ultrasound in daily clinical practice. For this, the review focuses on how the ultrasound image would be visualized in each of the pleuro-pulmonary pathologies to which it has access and its use in interventional pulmonology. In addition, a schematic illustration with the most frequent pa...

Novel therapeutics in hypertriglyceridaemia and chylomicronaemia.
Authors: García Díaz JD Abstract Currently there is evidence on hypertriglyceridaemia as an independent risk factor of atherosclerosis. Chylomicronaemia associated with very high concentration of triglycerides may cause severe and recurrent acute pancreatitis. The cause of most cases is a combination of a polygenetic basis with some lifestyles and pathological conditions. Some rare and familial chylomicronaemias are mendelian diseases with an autosomal recessive pattern. On the other hand, plasma triglycerides have considerable biological variability and usually descend with non-pharmacological interv...

Hip fracture mortality: Predictive models.
Authors: Pallardo Rodil B, Gómez Pavón J, Menéndez Martínez P Abstract The preoperative estimation of the risk of mortality after a hip fracture is very useful to plan time of surgery and perioperative care, inform patients and families about the prognosis and allows comparisons between different units. Different models have been developed to stratify mortality risk, but they show heterogeneity in terms of type of population and variables included, monitoring the time and statistical methods used, which makes it difficult to establish comparisons between them. The vast majority of them a...

Risk of infection associated with new therapies for the treatment of lymphoproliferative syndromes.
Authors: Los-Arcos I, Aguilar-Company J, Ruiz-Camps I Abstract Over the last decade, there have been important developments in the treatment of lymphoproliferative disorders. Apart from conventional chemotherapy, a wide array of therapies has been developed, with different indications. The aim of this review is to evaluate the risk of infection associated with these therapies, as well as establishing prevention recommendations. In all cases, the patient's underlying disease as well as concomitant or previous therapies have an impact on the risk of infection. Anti-CD20 antibodies (rituximab, ofatumumab and obinutuzu...
