A multi-modal approach to quality improvement: reducing 'door-to-ct' time in potenitally thombolysable stroke patients
Conclusion Early thrombolysis has been shown to improve outcomes in patients with ischaemic stroke.2 Reducing ‘Door-to-CT' scan time in the ED is an important step in enabling a reduction in the overall DTN time. This can be achieved by stream-lining key processes in the patient journey through the ED. A multi-modal approach ensured this change was introduced safely and effectively whilst re-enforcing a core change to standard working practice. We report an unusual case of recurrent stroke in a patient with vitamin B12 deficiency who was also homozygous for the methylene tetrahydrofolate reductase (MTHFR) gene mutation. The patient was a 35-year-old male vegetarian with no known medical history who initially presented with global aphasia, slurred speech, right facial weakness, and right-sided hemiplegia and was found to have a stroke (NIH Stroke Scale score of 25). Lateralization of Insular Ischemic Stroke is Not Associated With Any Stroke Clinical Outcomes: The Athens Stroke Registry
Background: Controversial evidence suggests that right insular stroke may be associated with worse outcomes compared to the left insular ischemic lesion. Perfusion computed tomography thresholds defining ischemic penumbra and infarct core: studies in a rat stroke model
ConclusionsThe data provide perfusion computed tomography defined relative cerebral blood flow thresholds for infarct core and ischemic penumbra within the first two hours after experimental stroke in rats. These thresholds were shown to be stable to define the volume of infarct core and penumbra within this time window.

The Detrimental Effect of Aging on Leptomeningeal Collaterals in Ischemic Stroke
Conclusion: Our findings show that there is a significant interplay between patient age and adequacy of leptomeningeal collateral circulation in patients with proximal MCA occlusion. The relationship could contribute to adverse tissue outcome and thereby to unfavorable clinical outcome observed in elderly patients with ischemic stroke.

Conclusions— When present in acute ischemic stroke, HAS indicates a high likelihood of arterial obstruction, but its absence indicates only a 50/50 chance of normal arterial patency.
