Sometimes a Difficult Decision to Swallow: Ethical Dilemmas When Patients with Dysphagia who Lack Capacity Want to Eat
Mr. J is an 82-year-old retired farmer with a preexisting diagnosis of moderate vascular dementia who recently suffered a stroke resulting in a new diagnosis of dysphagia and a loss of complex decision-making capacity. He is admitted to a hospital rehabilitation unit for a course of speech, physical and occupational therapy. His wife, Mrs. J, is his surrogate and has made clear that the couple's shared goals of care are for him to regain as much function as possible so he can return to his own home.

Virtual Reality in Today’s Healthcare Settings: An Analysis Using the Alberta Quality Matrix for Health
Virtual reality (VR) is emerging as a treatment modality across a variety of healthcare settings. Integrating a new technology as a form of therapy requires informed analysis to ensure that it is an effective, efficient, safe and valuable addition to the client experience. Using the Alberta Quality Matrix for Health as a framework, this detailed analysis examines the application of VR in pain management, mental health, stroke rehabilitation and palliative care. Through this multifocal lens, gaps are identified and a glimpse is provided into VR's potential for widespread adoption across healthcare settings.

Hospice Composition Based on Diagnosis is Associated with Caregiver-Reported Quality Measures
Conclusion: Hospices that cared for a greater proportion of dementia and stroke patients had poorer scores on caregiver-reported quality measures. Tube feeding in patients with dementia is associated with a longer palliative care unit stay
CONCLUSION: The present study showed that, in patients with an established diagnosis of dementia, TF is common and independently associated with a longer length of PCU stay. Improving Access to Specialist Palliative Care for Patients with Catastrophic Strokes: A Quality Improvement Project to Trigger Inpatient Palliative Care Consultations (QI730)
The American Stroke Association strongly recommends palliative care for patients hospitalized with catastrophic strokes to improve shared decision-making and relieve suffering. An automatic trigger to consider a consult for these patients may improve access to palliative care. The National Institutes of Health Stroke Scale (NIHSS) is a good predictor of short- and long-term outcomes and high scores (>= 20) are associated with poorer prognosis.

Outpatient Cardiac Palliative Care Reduces Healthcare Utilization (RP524)
The American Heart Association and American Stroke Association have put forth policy statements recommending early and continuous access to palliative care for patients with advanced heart disease. Early integration of palliative care has been proven to improve the quality of life of patients with advanced heart failure. Other studies have suggested that palliative care in general has cost savings to hospital systems.

Development of an indicator set for the evaluation of the population-based integrated healthcare model 'Gesundes Kinzigtal' (Healthy Kinzigtal).
CONCLUSION: On the basis of a broadly based research and participatory development process, a set of indicators has been developed that enables comprehensive evaluation of the regional quality of care of cross-indication, integrated care models focusing on common diseases. In order to be able to increasingly evaluate aspects of care coordination and patient orientation, health promotion as well as nursing, palliative and emergency care in the future, it would be helpful if routine data were collected or made accessible in these areas as well.
