Does my high blood pressure improve your survival? Overall and subgroup learning curves in health
Summary Learning curves in health are of interest for a wide range of medical disciplines, healthcare providers, and policy makers. In this paper, we distinguish between three types of learning when identifying overall learning curves: economies of scale, learning from cumulative experience, and human capital depreciation. In addition, we approach the question of how treating more patients with specific characteristics predicts provider performance. To soften collinearity problems, we explore the use of least absolute shrinkage and selection operator regression as a variable selection method and Theil–Goldberger mixed es...

Hospital heterogeneity: what drives the quality of health care
AbstractA major feature of health care systems is substantial variation in health care quality across hospitals. The quality of stroke care widely varies across NHS hospitals. We investigate factors that may explain variations in health care quality using measures of quality of stroke care. We combine NHS trust data from the National Sentinel Stroke Audit with other data sets from the Office for National Statistics, NHS and census data to capture hospitals ’ human and physical assets and organisational characteristics. We employ a class of non-parametric methods to explore the complex structure of the data and a set of c...

Study on COgnition and Prognosis in the Elderly (SCOPE): Design and Objectives.
Authors: Hansson L Abstract Hypertension is a major risk factor for cardiovascular events, including stroke, as well as for white matter lesions of the brain. There is also evidence to link cognitive impairment and dementia to hypertension. However, it has not been established whether antihypertensive treatment of elderly patients with a diastolic blood pressure (DBP) in the range 90-99 mmHg provides protection against major cardiovascular events and cognitive impairment. The Study on COgnition and Prognosis in the Elderly (SCOPE) is the first study designed to assess the effects of antihypertensive drug treatment ...

Economic evaluation of ezetimibe treatment in combination with statin therapy in the United States.
CONCLUSIONS: Compared with statin monotherapy, ezetimibe with statin therapy was cost-effective for secondary prevention of CHD and stroke and for primary prevention of these conditions in patients whose LDL-C level is ≥ 100 mg/dL and in patients with diabetes, taking into account a 90% cost reduction for ezetimibe. Vitamin C supplementation for the primary prevention of cardiovascular disease.
CONCLUSIONS: Currently, there is no evidence to suggest that vitamin C supplementation reduces the risk of CVD in healthy participants and those at increased risk of CVD, but current evidence is limited to one trial of middle-aged and older male physicians from the USA. There is limited low- and very low-quality evidence currently on the effect of vitamin C supplementation and risk of CVD risk factors. PMID: 28301692 [PubMed - as supplied by publisher]

War during Childhood: The Long Run Effects of Warfare on Health
Publication date: Available online 7 March 2017 Source:Journal of Health Economics Author(s): Mevlude Akbulut-Yuksel This paper estimates the causal long-term consequences of an exposure to war in utero and during childhood on the risk of obesity and the probability of having a chronic health condition in adulthood. Using the plausibly exogenous city-by-cohort variation in the intensity of WWII destruction as a unique quasi-experiment, I find that individuals who were exposed to WWII destruction during the prenatal and early postnatal periods have higher BMIs and are more likely to be obese as adults. I also find an eleva...

Long-term care provision, hospital bed blocking, and discharge destination for hip fracture and stroke patients
AbstractWe examine the relationship between long-term care supply (care home beds and prices) and (i) the probability of being discharged to a care home and (ii) length of stay in hospital for patients admitted to hospital for hip fracture or stroke. Using patient level data from all English hospitals and allowing for a rich set of demographic and clinical factors, we find no association between discharge destination and long-term care beds supply or prices. We do, however, find evidence of bed blocking: hospital length of stay for hip fracture patients discharged to a care home is shorter in areas with more long-term care...

The impact of post-procedural complications on reimbursement, length of stay and mechanical ventilation among patients undergoing transcatheter aortic valve implantation in Germany
ConclusionsPost procedural complications such as bleeding events, acute kidney injuries and strokes are associated with increased resource use and substantial amounts of additional reimbursement in Germany, which has important implications for decision making outside of the usual clinical sphere.

Estimated burden of cardiovascular disease and value-based price range for evolocumab in a high-risk, secondary-prevention population in the US payer context.
CONCLUSION: Real-world CVD burden is substantial. Using the observed CVD burden in CPRD and the CTTC relationship, the cost-effectiveness analysis showed that, accounting for uncertainties, the expected value-based price for evolocumab is higher than its current annual cost as long as the payer discount off list price is greater than 20%.
