Transcutaneous Auricular Vagus Nerve Stimulation-Paired Rehabilitation for Oromotor Feeding Problems in Newborns: An Open-Label Pilot Study
Neonates born premature or who suffer brain injury at birth often have oral feeding dysfunction and do not meet oral intake requirements needed for discharge. Low oral intake volumes result in extended stays in the hospital (>2 months) and can lead to surgical implant and explant of a gastrostomy tube (G-tube). Prior work suggests pairing vagus nerve stimulation (VNS) with motor activity accelerates functional improvements after stroke, and transcutaneous auricular VNS (taVNS) has emerged as promising noninvasive form of VNS. Pairing taVNS with bottle-feeding rehabilitation may improve oromotor coordination and lead to ...

Clinical outcomes of moderate to severe acute ischemic stroke in a telemedicine network
The objective of this study was to analyze clinical and imaging based outcomes of moderate to severe stroke following treatment with thrombolysis, thrombectomy or a combination of both in a telemedicine network. Data of this retrospective study was abstracted from the institutional telestroke database. Patients with a National Institute of Health Stroke Scale score (NIH-SS ≥ 10) were included into the study. Primary outcome measure was the difference in NIH-SS at admission compared to discharge from the hospital. Secondary outcome measure was the discharge disposition defined as favorable (discharge to home or rehabi...

Circulating Aquaporin-4 as a biomarker of early neurological improvement in stroke patients: a pilot study
Publication date: Available online 28 October 2019Source: Neuroscience LettersAuthor(s): Laura Ramiro, Alba Simats, Anna Penalba, Alvaro Garcia-Tornel, Alex Rovira, Fernando Mancha, Alejandro Bustamante, Joan MontanerAbstractPatients’ outcome prediction after ischemic stroke is still challenging. Aquaporin-4 (AQP4) is a water channel that is up-regulated in the brain after the ischemic event, but its presence in bloodstream of stroke patients has not been previously studied. The aim of this pilot study was to investigate circulating AQP4 levels after stroke and its correlation with infarct growth and neurological outcome...

Frequency, Risk Factors, and Clinical Significance of Incorrect Dose of Alteplase Due to Weight Estimation in Acute Ischemic Stroke Patients: A Single-Center Retrospective Study
Stroke is a clinical emergency requiring urgent recombinant tissue-type plasminogen activator treatment in eligible patients. The dosage of thrombolytic agent (alteplase) is weight dependent. However, many patients receive thrombolytic therapy based on weight estimation. Here, we assess the frequency of incorrect thrombolytic therapy dose as a result of weight estimation and evaluate the short-term safety and efficacy of alteplase misdose. Of 237 patients, weight was estimated in 147 (62%), of which 33 patients (22.4%) were treated with an erroneous dose of alteplase. An incorrect dose was associated with neither poor outc...

Retrospecting atrial fibrillation and stroke severity: impact onset time of acute ischemic stroke.
Abstract Irrespective of the duration of the therapeutic window, the prognosis for acute ischemic stroke is closely correlated with the time of treatment initiation. Time from symptom onset to hospital arrival, defined as the onset time, is the determining parameter. Besides the prehospital transport, whether relative factors of illness may have an impact on stroke onset time is uncertain, which were invested in this study through statistical analyses. Results show that relative factors of illness, such as history of atrial fibrillation and stroke severity, impact stroke onset time. PMID: 31321960 [PubMed - in process]

Treatment With Intravenous Alteplase for Acute Ischemic Stroke After Reversal of Dabigatran With Idarucizumab: A Case Study
Treatment options for anticoagulated patients presenting with ischemic stroke are limited. Off-label use of idarucizumab to rapidly reverse the anticoagulant effect of dabigatran may ensure eligibility for thrombolytic therapy with alteplase. This case describes a 77-year-old white male who presented to the hospital 89 minutes after sudden onset of right-sided hemiparesis, dysarthria, and facial palsy. Significant history included atrial fibrillation and previous right-sided cortical stroke. Medication reconciliation revealed he was taking dabigatran 150 mg twice a day, with the last dose being 179 minutes before presentat...

Risk factors for ischemic stroke post bone fracture
Publication date: Available online 7 November 2018Source: Journal of Clinical NeuroscienceAuthor(s): Qi An, Zhe Chen, Kang Huo, Hua Su, Qiu-Min QuAbstractStroke is one of the most devastating complications after bone fracture. However, due to the rarity of the complication, the risk factor for post fracture stroke remains unknown. We retrospectively reviewed 2914 fractured adults referred to the first affiliated hospital of Xi’an Jiaotong University, a regional referral center of China, from January 2008 to May 2013. As a result, among the 2914 patients, 13 of them had newly onset stroke within a median of 4 days after...

Iron nanoparticle contrast enhanced microwave imaging for emergent stroke: A pilot study
Publication date: Available online 1 November 2018Source: Journal of Clinical NeuroscienceAuthor(s): Joseph S. Hudson, Timothy K. Chung, Benjamin S. Prout, Yasunori Nagahama, Madhavan L. Raghavan, David M. HasanAbstractEmergent stroke is mostly evaluated using hospital based imaging. Quick imaging allows for rapid administration of IV thrombolysis and outcome improvement. Microwave imaging (MI) is an emerging portable imaging modality. Iron oxide nanoparticles are known to interact with microwave frequency electromagnetic radiation. In this manuscript, we provide proof of concept for a novel iron oxide nanoparticle enhance...

Serum 25-hydroxyvitamin D deficiency predicts poor outcome among acute ischemic stroke patients without hypertension
Publication date: Available online 3 May 2018Source: Neurochemistry InternationalAuthor(s): Bingjun Zhang, Yuge Wang, Yi Zhong, Siyuan Liao, Zhengqi LuAbstract25-Hydroxyvitamin D (25(OH)D) deficiency is a frequent condition in patients who suffer acute ischemic stroke (AIS), and several studies suggested that it may be associated with a poorer prognosis. Whether this association is affected by hypertension is unclear. Our aim was to investigate the association between 25(OH)D levels and both clinical severity and outcome after 3 months in AIS patients stratified by the history of hypertension. Consecutive first-ever AIS pa...
