The Impact of Longitudinal Data-Completeness of Electronic Health Record Data on the Prediction Performance of Clinical Risk Scores
Clin Pharmacol Ther. 2023 Apr 7. doi: 10.1002/cpt.2901. Online ahead of print.ABSTRACTThe impact of electronic health record (EHR) discontinuity, i.e., receiving care outside of a given EHR system, on EHR-based risk prediction is unknown. We aimed to assess the impact of EHR-continuity on performance of clinical risk scores. The study cohort consisted of patients aged ≥65 years with ≥1 EHR encounter in the two networks in Massachusetts (MA, 2007/1/1-2017/12/31, internal training and validation dataset), and one network in North Carolina (NC, 2007/1/1-2016/12/31, external validation dataset) that were linked with Medica...

Impact of emergency department arrival time on door-to-needle time in patients with acute stroke
ConclusionFactors that contribute most to a delay in DTN time include arrival during the night shift, lack of a dedicated stroke team, longer time-to-CT read, and arrival as a walk-in. All of these are addressable factors from an operational standpoint and should be considered when performing quality improvement of hospital protocols.

Common clinical blood and urine biomarkers for ischemic stroke: an Estonian Electronic Health Records database study
ConclusionsWe conclude that the EHR database and the risk factors uncovered are valuable resources in screening the population for risk of IS as well as constructing disease risk scores and refining prediction models for IS by ML.

A Real-World Exploration into Clinical Outcomes of Direct Oral Anticoagulant Dosing Regimens in Morbidly Obese Patients Using Data-Driven Approaches
ConclusionData-driven approaches can identify key factors associated with clinical outcomes following the dosing of DOACs in morbidly obese patients. This will help design further studies to explore well tolerated and effective DOAC doses for morbidly obese patients.

Outcome of transcatheter atrial septal defect closure in a nationwide cohort
CONCLUSION: After TC closure of ASD, patients had a higher risk of new-onset atrial fibrillation and migraine than controls without ASD. As novel findings, we found an increased risk for ischemic heart disease, AV conduction disorders, and ventricular fibrillation/tachycardia.Key messagesEven though patients have an excellent overall prognosis after percutaneous ASD closure, the increased incidence of major comorbidities like atrial fibrillation and heart failure prompts more thorough lifelong follow-up.This study's novel findings revealed the increased risk for ischemic heart disease, AV conduction disorders, or ventricul...

A Learning Health System Infrastructure for Precision Rehabilitation After Stroke
We describe the creation of a Precision Rehabilitation Data Repository that facilitates access to systematically collected data from the electronic health record as part of a learning health system to drive precision rehabilitation. Specifically, we describe the process of (1) standardizing the documentation of functional assessments, (2) obtaining regulatory approval, (3) defining the patient cohort, and (4) extracting data for the Precision Rehabilitation Data Repository. The development of similar infrastructures at other institutions can help generate large, heterogeneous data sets to drive poststroke care toward preci...

Clinical impact of the infrapatellar location in symptomatic peripheral arterial disease patients
CONCLUSION: Infrapatellar disease was anindependent factor for increased hospitalization of CLTI, major and minor amputations events, compared to suprapatellardisease in symptomatic revascularized PAD patients.PMID:36480817 | DOI:10.24875/ACM.22000161
