E-094 Higher intracranial positioning of 8fr-guide catheter improves efficacy of aspiration thrombectomy in large vessel occlusion stroke
ConclusionPositioning a large bore guide catheter within the petrous segment or further distal resulted in significantly higher rates of first pass effect, lower procedural times, lower total number of passes, and better rates of excellent recanalization.Disclosures M. E-097 Acute ischemic stroke thrombus proteome is associated with time since last known well
ConclusionsThis study points, for the first time, to differences in the proteomic landscapes between thrombi from AIS patients that are related to time since symptom onset. Taken together, the present data support a view of the stroke thrombus as a living, dynamic microenvironment with increasing intracellular protein and RNA processing with increased tLKW. More work is required to further characterize early and late stroke thrombi, as a changing clot landscape may directly underlie responsiveness to treatment.Disclosures S. Akkipeddi: None. R. E-098 Initial experience with the pipeline vantage flow diverter - vantage one study: six-month results, technical and clinical consideration
ConclusionThe new PVED reveals improved device visibility, ease of delivery and repositioning with precise distal opening in comparison to previous PED generations. Initial results demonstrate overall good device performance, safety profile and effectiveness. The implant requires a modified deployment technique for successful vessel wall apposition of the proximal stent as opposed to the previous generations of PED.ReferencesLylyk I, Scrivano E, Lundquist J, Ferrario A, Bleise C, Perez N, Lylyk PN, Viso R, Nella-Castro R, Lylyk P. E-100 Rescue therapy with self-expandable stenting versus medical therapy after failed mechanical thrombectomy for acute large vessel occlusion stroke: a systematic review and meta-analysis
ConclusionOur study showed that, when compared to patients treated with best medical management, patients who received rescue therapy with SES after failed mechanical thrombectomy had a higher chances of functional independence, with lower mortality and similar sICH. However due to serious risk of bias and significant heterogeneity in the studies included, randomized trials using a standard technique and independently adjudicated outcomes are needed to confirm our results.Disclosures J. E-103 Transradial carotid stenting using a 6 french simmons-2 guide catheter in comparison to transfemoral approach: a single center experience
ConclusionsTransradial carotid stenting using lower profile technique appears to be a safe, effective, and faster option for carotid revascularization compared to traditional transfemoral approach although conversion rate may be high which can be mitigated through proper case selection.Disclosures M. E-104 Inhibiting the neuron-immune axis in acute ischemic stroke reverses microglia-induced neuroinflammation after reperfusion
ConclusionTogether, these findings indicate that ischemic/hypoxic-induced neuronal excitotoxicity promotes neuroinflammation in the setting of reperfusion after ischemic stroke. Inhibiting abnormal neuronal activation triggered by ischemia may prevent microglia-induced neuroinflammation that could protect the brain from secondary brain damage.Disclosures X. Guo: None. D. E-105 Validation of rapid noncontrast stroke CT platform for ICH and LVO detection
ConclusionThe RAPID NCCT Stroke platform demonstrated superior performance to radiologists for detecting LVO from a NCCT. This could lead to earlier identification of LVO and faster treatment times. E-107 Carotid artery stenting in patients with symptomatic non-stenotic carotid artery disease - a case series
ConclusionsIn this case series performed over a 10-year period at a high-volume stroke center we show that CAS is safe and efficient in patients with SyNC. Previous studies have established safety and effectiveness of CES in patients presenting with SyNC, however there is a paucity of reports in the literature regarding the effect of CAS in this patient population. Recent guidelines have brought specific attention to high-risk plaque characteristics which suggest that plaque instability plays a major role in the onset of ischemic events, regardless of lumen narrowing. Advanced imaging techniques, such as Optical Coherence ...

E-108 Endovascular treatment of patients with acute ischemic stroke due to underlying arterial dissection: a nationwide analysis

E-113 Lower low-density lipoprotein level increases the risk of delayed parenchymal hematoma following endovascular thrombectomy
ConclusionLDL level <50 mg/dL independently predicted delayed PH following thrombectomy and LDL >100 was shown to be a protective factor, irrespective of statin use. Further study is needed to explore the underlying mechanism and identify the strategies to prevent post-thrombectomy delayed PH among patients with low LDL levels.Disclosures S. Ahn: None. S.
