E-257 Flow diversion of the ophthalmic aneurysms, outcomes in relation to the ophthalmic artery origin
ConclusionFlow diversion is safe and effective for carotid ophthalmic aneurysms treatment. Adjunct coil embolization and ophthalmic artery origin in relation to the aneurysm sac are independent factors that can influence treatment result, and should be considered when planning treatment.Disclosures M. E-260 Bilateral cerebral arteriovenous shunting through pial and perforating vessels with multiple strokes and intraparenchymal hemorrhages in a patient with hepatopulmonary syndrome
This study presents a unique case of a 54-year-old male patient with a history of stroke, liver cirrhosis, portal vein thrombosis, hypertension, diabetes, and bladder cancer, who experienced multiple episodes of intracranial hemorrhages, stroke and worsening confusion over the past five years. The patient‘s clinical presentation raised suspicion for an underlying occult vasculopathy. Digital subtraction angiography (DSA) revealed multiple curvilinear, irregular, bilateral pial vessels with early high-flow arteriovenous shunting, consistent with vascular shunting that is associated with liver cirrhosis. The patient&ls...

E-261 Variability of response on prophylactic prasugrel for endovascular treatment of intracranial aneurysms: clinical implications
ConclusionIn the neurointerventional field, high BMI and prior use of antithrombotic agents were related to HTPR, and low BMI was associated with LTPR on prophylactic prasugrel. High hematocrit levels decreased the risk of HTPR. When preparing endovascular treatment for intracranial aneurysms, attention to patients with these clinical features is required to address the possibility of ischemic or bleeding complications.Abstract E-261 Table 1Risk factor analysis of high on-treatment platelet reactivity (HTPR) Variables Univariate Analysis Multivariate Analysis OR (95% CI) P OR (95% CI) P Age 1.02 (0.97-1.06) 0.53 Female o...

E-262 Pipeline embolization device for the treatment of unruptured intracranial saccular aneurysms: a systematic review and meta-analysis of long-term outcomes
ConclusionThe PED is a safe and effective method to treat intracranial saccular aneurysms: The long-term complete occlusion rate is high (81%), with almost a quarter of the patients persist with residual filling. Even longer follow-ups are expected to show higher occlusion rates.Disclosures A. Ghaith: None. J. Rios-Zermeno: None. E. Greco: None. M. Bydon: None. R. Tawk: None.

E-275 Endovascular therapy for acute stroke with a large ischemic region: a systematic review and meta-analysis
ConclusionOur study suggests that EVT might be an effective and relatively safe treatment option for the treatment of patients with large core infarcts, although more large-scale trials are needed to consolidate the results and to make inclusion criteria and the patient selection process clearer.Disclosures A. European Stroke Organisation (ESO) - European Society for Minimally Invasive Neurological Therapy (ESMINT) Guidelines on Mechanical Thrombectomy in Acute Ischemic Stroke
Conclusions MT is the standard of care in patients with LVO-related acute stroke. Appropriate patient selection and timely reperfusion are crucial. Further randomized trials are needed to inform clinical decision-making with regard tothe mothership and drip-and-ship approaches, anesthaesia modalities during MT, and to determine whether MT is beneficial in patients with low stroke severity or large infarct volume.

Intravenous thrombolysis before mechanical thrombectomy in patients with atrial fibrillation
The introduction into clinical practice of endovascular mechanical thrombectomy (MT) has brought better results in patients with ischemic stroke from large vessel occlusion (LVO) in the anterior circulation in whom therapy is started 4.5–5 hours after symptoms onset.1–3 Controversies exist about the usefulness of intravenous thrombolysis (IVT) before MT in patients with atrial fibrillation (AF). AF is the probable etiology in >25% of ischemic strokes, and >40% of patients with ischemic stroke from LVO in the anterior circulation have a clinical history of AF.4 5 AF is an independent predictor of poor func...

Slow is smooth, smooth is fast
Modern infantry combat motto. If you can’t move, you get pinned down but if you move too fast you get outflanked and surrounded. The milestone trials1 in 2015 showed that patients with large vessel occlusion can be effectively treated to prevent future disability and, to a certain extent, their mortality. Large vessel occlusion thrombectomy opened the doors of a new era in neurointerventional surgery, in which small departments, often handled by one or two operators, had to pivot and face a steady, continuous increase in the number of procedures two- or threefold their usual capacity. Stroke can also be held accounta...

Withdrawn: Effect of routing paradigm on patient-centered outcomes in acute ischemic stroke
This article has been withdrawn from the Journal of NeuroInterventional Surgery website. The editorial office has received an email from Dr Akash Kansagra, one of the authors, detailing multiple corrections in their data specifically pertaining to errors of over-representation in their simulation analysis. Although the authors have stated that these data errors do not alter the conclusions of their manuscript, the editorial board has withdrawn this paper and asked the authors to address these errors in a revision that would be subject to further peer review.

Time-to-treatment with endovascular thrombectomy in patients with large core ischemic stroke: the 'late window paradox
The results of the recently published randomized clinical trials of endovascular thrombectomy (EVT) for large vessel occlusion (LVO) in acute ischemic stroke (AIS) patients with a large ischemic core: ANGEL-ASPECT,1 SELECT-2,2 and the RESCUE-Japan LIMIT trials,3 are promising and are expected to lead to more AIS-LVO patients undergoing EVT. Prior studies have demonstrated that shorter time-to-treatment has been found to be directly associated with better functional outcomes in AIS-LVO patients undergoing EVT.4 In this short commentary, we have pooled the available data from the abovementioned trials to conduct a subgroup a...

National reduction in cerebral arteriovenous malformation treatment correlated with increased rupture incidence
Conclusions After 2014, the likelihood of intervention for unruptured AVMs decreased while the incidence of ruptured AVMs increased. These findings suggest that fewer unruptured AVM treatments may lead to increases in AVM rupture incidence.
