E-126 Multicenter flow diversion in 200 patients for treatment of intracranial aneurysms
ConclusionThis report characterizes patient demographics, aneurysm characteristics, procedure complications and long-term angiographic and clinical outcomes for a large patient population across multiple medical centers treated with flow diversion. Overall, procedures resulted in a high rate of occlusion and low rate of serious complications. Continued iterative outcomes analysis is warranted particularly as flow diversion technology evolves. Despite diverse hospital settings, differences in presentation, patient characteristics and the inclusion of off-label treatments, flow diversion procedures remained safe and effectiv...

E-129 Trends in demographic and geographic disparities of stroke-related mortality in older adults in the United States from 1999 to 2020
Stroke is a leading cause of mortality and morbidity in the United States. Many risk factors are controversial, and their relative importance is not fully understood. Elucidating the association between demographic and geographic disparities and stroke-related mortality in older adults will guide policy to alleviate stroke burden. This retrospective cohort study characterizes stroke-related mortality among individuals aged 55 and older in the United States from January 1999 to December 2020 using the Centers for Disease Control and Prevention Wide-Ranging Online Data for Epidemiologic Research database. Segmented regressio...

E-135 Dejerine-roussy syndrome in the setting of right sigmoid sinus thrombosis: a unique case report
DiscussionThis case illustrates the important clinical aspects and physical findings needed to diagnose DRS. The presence of focal allodynia and hyperalgesia with recent cerebrovascular incidents provides significant evidence for DRS. The purpose of this case is to provide awareness and to increase clinical suspicion of DRS during examination, especially in the setting of cerebral venous sinus occlusion as infarction alone may not present as atypical allodynia.Disclosures S. E-142 A new graphical interface to capture quantitative venous outflow metrics
ConclusionsOur newly developed graphical interface allows users to output quantitative metrics that may represent AIF and VOF. These metrics should be checked against existing scales for association and performance.Abstract E-142 Figure 1Disclosures G. Adusumilli: None. S. Christensen: 2; C; iSchemaView. 4; C; iSchemaView. E-145 Mortality following mechanical thrombectomy for ischemic stroke in patients with COVID-19
ConclusionOverall, this study indicates that COVID-19 is a predictor of mortality among mechanical thrombectomy. This finding is likely multifactorial but may be related to multisystem inflammation, hypercoagulability, and re-occlusion seen in COVID-19 patients. Treatment-related factors, such as delays in care, may also play a role. Additional research would be needed to further delineate these findings.Disclosures J. Dallas: None. T. Wenger: None. K. E-150 Mechanical thrombectomy in distal middle cerebral artery occlusions using a novel blind exchange mini-pinning technique: experience in 7 consecutive cases
ConclusionBlind mini-pinning allows for combined use of distal stentriever with a 035 thrombectomy catheter, affording a larger ID than traditional microcatheter engagement of the thrombus with enhanced stability for removal, and improved reperfusion rates in distal occlusions. This technique may be efficacious for the treatment of distal occlusions and further studies are needed to compare outcomes in distal occlusions.Abstract E-150 Figure 1(A) Deployment and blind mini-pinning navigation of 035 aspiration catheter (blue) through 071 (black) and 088 thrombectomy catheter (orange) into the M2 segment with follow-up angiog...

E-151 Efficacy of an incidental aneurysm alert system (IAAS) and the associated disparities of incidentally detected intracranial aneurysms
ConclusionsIAAS is an effective alerting system and can improve the management of incidentally discovered cerebral aneurysms. The natural history of cerebral aneurysms may differ for Hispanic andAsian Americans, suggesting a potentially higher risk at presentation. Future research is needed to better understand these disparities’ causes and reduce their risks.Disclosures K. Fukuda: None. Y. Ghochani: None. D. Enzmann: None. C. Arnold: None. X. Chen: None. J. E-152 Cone-beam CT angiogram acquisition in the angiography suite for LVO detection in acute ischemic stroke
ConclusionDTAS triage workflow with cone-beam head CT and CTA image acquisition in the angiography suite may be a next step to further optimize LVO stroke pathways by bypassing conventional CT/MR. This new workflow has the potential to further decrease door-to-needle times and thereby may improve patient outcome.Disclosures A. Kuhn: None. J. Singh: None. M. Garcia: None. S. Sarid: None. M. E-153 Mechanical thrombectomy for patients older than 80 years old with large vessel occlusion stroke: systematic review and meta-analysis
ConclusionOur results suggest that MT in the elderly population is effective to achieve functional independence. Our data was unable to provide information about 90-days mortality, all ICH and sICH. Given our limitations, a RCT comparing MT versus MM in this population would be needed to confirm our results. However, including elderly patients in RCTs is always challenging. So, our results could provide support in the real world to offer this highly effective treatment to this challenging population.Disclosures J. Sequeiros: None. M. Cornejo: None. F. Terry: None. C. E-161 Transradial arterial access for neurointervention: the basics, benefits, and tools
Transradial access (TRA) is a feasible method for interventional neurovascular procedures, gaining popularity after becoming the standard access route in cardiovascular interventions. While advantages of TRA include better safety, cost reduction, and patient experience, barriers include a narrow variety of dedicated equipment for radial to neurovascular target vessels, need for further training for physicians and fellows, and physician bias toward Transfemoral access (TFA) due to training and familiarity. This educational poster focuses on the general technical details of TRA, including discussion of the TRA and distal TRA ...

E-162 Procedural predictors of early neurologic improvement following endovascular thrombectomy in patients with large vessel occlusion
ConclusionIn a large retrospective database, we found that excellent recanalization, defined as TICI 2C or above, significantly predicted ENI and delta NIHSS of 6 or above within 24 hours of procedure in patients with acute ischemic stroke from a large vessel occlusion.Disclosures M. E-164 Symptomatic developmental venous anomaly thrombosis associated with covid-19 infection
We describe a milder case of DVA thrombosis in the setting of COVID-19 infection presenting with unilateral hemiparesis requiring medical management alone. A 41-year-old female with hypertension who presented with left hemiparesis. Non-contrast computed tomography (CT) of head revealed linear calcifications in the right centrum semiovale consistent with DVA. Magnetic resonance imaging (MRI) revealed an acute ischemic stroke in the right corona radiata adjacent to the vascular abnormality. Extensive workup for other causes of ischemic stroke was unrevealing. Cerebral angiography ruled out arteriovenous shunting and did not ...

E-166 The effect of covid-19 on treatment and outcomes following ischemic stroke: a national assessment
ConclusionThis study identified a relationship between COVID-19 diagnosis and worse outcomes for each metric assessed. Most notably, even when controlling for disease-related factors, patients with COVID-19 were significantly less likely to undergo mechanical thrombectomy. Further, COVID-19 was associated with increased in-hospital mortality and worsened discharge disposition, although the exact causal relationship of this is unclear. Several factors might underly this, ranging from systemic/multisystem inflammation and worsened disease severity to logistical barriers to treatment caused by COVID-19. Further research is ne...
