E-224 Outcomes of mechanical thrombectomyin stroke patients with extreme large infarction core
ConclusionAbout one in five patients presenting with extreme large infarction core (ASPECTS 0-2) may achieve a favorable 90-day functional outcome following MT. However, MT in this group of patients was associated with high mortality and sICH rate.Disclosures E. Almallouhi: None. M. Anadani: None. S. Samir Elawady: None. M. Mahdi Sowlat: None. I. E-226 Endovascular treatment of pediatric arteriovenous shunts
ConclusionEndovascular treatment of pediatric arteriovenous shunts is extremely difficult and associated with high risks of complications. However, excellent clinical results can be achieved in specialized clinics. The choice of embolic agent should depend on the hemodynamic load of the pathological blood flow.Disclosures V. E-228 Single center experience using 3mm trevo stent retrievers in medium vessel occlusion thrombectomy
ConclusionUse of 3mm Trevo NXT stent retriever in medium vessel occlusion for first line and rescue thrombectomy in our single center experience demonstrated an adequate safety and efficacy profile, thus endorsing its use in mechanical thrombectomy applications beyond large vessel occlusion cases. Future multicenter studies will build upon this initial experience to further characterize the utility of novel stent retrievers in medium vessel occlusions.Disclosures C. Zhao: None. M. E-229 Mechanical thrombectomy in low alberta stroke program early CT score (ASPECTS) in hyperacute stroke - a systematic review and meta-analysis

E-231 Long-term safety and efficacy of the pipeline embolization device for treatment of small vs. large aneurysms: a systematic review and meta-analysis
ConclusionThe long-term occlusion rate after PED treatment is higher in large vs. small aneurysms. Symptomatic thromboembolic rates with stroke are also higher in large vs. small aneurysms. The difference in the rates of aneurysm rupture and intracranial hemorrhage was insignificant. Although the PED seems a safe and effective treatment for small and large aneurysms, further studies are required to clarify how occlusion rate and morbidity are affected by aneurysm size.Disclosures A. E-232 Multicenter experience with the route 92 medical freeclimb 88 catheter for aspiration thrombectomy of supraclinoid and middle cerebral artery M1 occlusions
ConclusionsOur initial experience with a novel full length SLBC (Route 92 Freeclimb 88 catheter) and its customized catheter delivery device (Tenzing 8) for aspiration thrombectomy of SCICA and M1 LVOs demonstrated successful delivery to the target occlusions in 93% of patients, with first pass mTICI 2c/3 achieved in 86%. E-236 Automated pre- and post-operative volumes estimates risk of retreatment in chronic subdural hematoma: a retrospective, multi-center study
Conclusions/RelevanceLarger pre- and post-operative cSDH volumes increase the risk of cSDH retreatment. Volume thresholds may allow identification of patients at high risk of cSDH retreatment who would benefit from adjunct treatments. Machine learning algorithm can quickly provide accurate estimates of pre and post operative volumes.Disclosures J. E-240 Endovascular thrombectomy beyond 24 hours of last known well: a systematic review and meta-analysis

E-242 Successful mechanical thrombectomy through a frozen elephant aortic arch graft via radial approach
We report the first published case of a mechanical thrombectomy for acute ischemic stroke via radial access in a patient with status post recent frozen elephant trunk graft (FET) for aortic arch repair. A 69 year-old male presented with a left middle cerebral artery large vessel occlusion two weeks after FET graft repair. After consultation with the patient’s cardiothoracic surgeon, a radial approach was deemed safest to access the left-sided large vessel occlusion. The FET procedure is a hybrid open and endovascular repair of aortic arch and descending aortic aneurysm and dissection repair. A vascular graft is used ...

E-245 Association of pre-treatment cerebral microbleeds with hemorrhagic transformation following successful endovascular recanalization in patients with acute ischemic stroke arising from large artery occlusion
ConclusionsThese findings suggest that the presence of CMBs is associated with HT and poor functional outcome following EVT and had no effect on mortality in patients with successful endovascular recanalization. However, CMBs do not show such a high impact on HT as other literature in our study. Therefore, it seems appropriate to consider CMB status in individual risk stratification of poor clinical outcome following recanalization by EVT with AIS. Future large and well-designed prospective studies are required to better characterize the underlying factors behind CMB-associated clinical deterioration.Abstract E-245 Figure ...

E-247 The universal effect of the 'golden hour on stroke thrombectomy outcomes
ConclusionsIn this work we demonstrate the universality of the ‘Golden Hour’ effect, in which procedures lasting more than 1 hour are associated with worse clinical outcomes and higher rates of symptomatic hemorrhage. This study will assist clinicians in determining when to be more aggressive in their approach when aiming for TICI 3 reperfusion in different subpopulations of patients.Disclosures M. Ash: None. L.
