Recent studies have expanded the window for the use of systemic and catheter-directed thrombolysis and mechanical thrombectomy for acute stroke management. The purpose of this review was to evaluate the impact that contemporary cerebral imaging has on the indications for emergent cervical carotid revascularization.

LEA 24. Synthetic Marijuana and Acute Lower Limb Thromboembolism and Ischemia —A Case Report
Synthetic marijuana use has been reported in the last years as a possible causative factor of different cardiovascular events, including myocardial infarction, ischemic stroke, intracranial bleeding, and cerebral vasospasm. One case of aortic thrombosis was also reported, but that was in a patient using cocaine and synthetic marijuana together. A case of lower limb thromboembolism and synthetic marijuana use has not been reported to date. Intoxication, material impurity, blood vessel reactivity, and chemical interaction with other drugs have been proposed as possible mechanisms of these events.

Misconceptions regarding the adequacy of best medical intervention alone for asymptomatic carotid stenosis
Medical intervention (risk factor identification, lifestyle coaching, and medication) for stroke prevention has improved significantly. It is likely that no more than 5.5% of persons with advanced asymptomatic carotid stenosis (ACS) will now benefit from a carotid procedure during their lifetime. However, some question the adequacy of medical intervention alone for such persons and propose using markers of high stroke risk to intervene with carotid endarterectomy (CEA) and/or carotid angioplasty/stenting (CAS).

Alternative ultrasound modalities to attempt to assess high risk asymptomatic carotid plaques
Duplex ultrasound examination, computed tomography scans, and magnetic resonance angiography have traditionally been used as noninvasive methods to assess carotid stenosis severity. The degree of stenosis in asymptomatic patients has often been used as the main criterion to assess risk for plaque embolization and stroke. There has been an effort to find alternate methods to assess vulnerable plaques. This has included many modalities, including magnetic resonance imaging and other ultrasound techniques.

Carotid endarterectomy after systemic thrombolysis in a stroke population
Vascular specialists are increasingly being requested to perform carotid endarterectomy (CEA) after intravenous thrombolysis (IVT) for stroke patients, raising concerns about hemorrhagic complications. Few case series and registry reports have assessed the question, and even fewer studies have included a control group. The aim of this study was to evaluate the overall outcome of patients undergoing CEA after IVT and to compare them with contemporary patients with CEA after simple stroke (non-IVT group).

The Vascular Quality Initiative 30-day stroke/death risk score calculator after transfemoral carotid artery stenting
Carotid artery stenting (CAS) was introduced as an alternative carotid revascularization procedure in patients deemed to be at high risk for carotid endarterectomy. Although techniques and selection criteria for patients have dramatically improved, CAS continues to have higher risk of stroke and death in comparison to carotid endarterectomy. Several risk factors are known to be associated with worse outcomes. Whereas knowledge of these independent factors is helpful, clinical decision-making is further refined when these are considered in aggregate.

Optimal closure technique of the arteriotomy after carotid endarterectomy
The optimal closure technique of the arteriotomy after carotid endarterectomy (CEA) has been the subject of extensive debate. A recent systematic review and meta-analysis compared outcomes after CEA with patch vs primary closure (N  = 29 studies; 9 randomized controlled trials [RCTs] and 20 non-RCTs; n = 13,219 CEAs).1 Primary closure was associated with a higher 30-day stroke rate compared with patch closure when both RCTs and non-RCTs were pooled together (odds ratio [OR], 1.9; 95% confidence interval [CI], 1.2-2.9) as wel l as when non-RCTs alone were considered (OR, 1.9; 95% CI, 1.1-3.3).

Discordant knowledge about atherosclerosis disease among French general practitioners and residents
We read with interest the paper entitled “Knowledge gap of peripheral artery disease starts in medical school” from AlHamzah and colleagues.1 In their study of 72 graduating medical students, the authors found that students have a suboptimal knowledge of coronary artery disease (CAD) and lower extremity peripheral artery disease (PAD). In view of the small number of students, readers may doubt the generality of these results. However, we would like to underscore that we found similar results in two studies in France using a national survey with three clinical cases: one about CAD, one about ischemic stroke, and one about PAD.

Delayed Carotid Endarterectomy After Admission in Symptomatic Carotid Artery Disease Is Associated With Lower Postoperative Stroke Rates in the Medicare Population
The appropriate timing of carotid endarterectomy (CEA) in symptomatic carotid artery disease is still controversial. Despite guideline recommendations to perform CEA within 14 days, recent studies have favored delayed CEA to decrease the risk of postoperative stroke. The goal of this study was to evaluate the timing of CEA for symptomatic disease in the Medicare population and its effects on postoperative stroke rates.
