Risk of Stroke before Revascularisation in Patients with Symptomatic Carotid Stenosis: A Pooled Analysis of Randomised Controlled Trials
Current guidelines recommending rapid revascularisation of symptomatic carotid stenosis are largely based on data from clinical trials performed at a time when best medical therapy was potentially less effective than today. The risk of stroke and its predictors among patients with symptomatic carotid stenosis awaiting revascularisation in recent randomised controlled trials (RCTs) and in medical arms of earlier RCTs was assessed.

Society for vascular surgery clinical practice guidelines for management of extracranial cerebrovascular disease
Management of carotid bifurcation stenosis in stroke prevention has been the subject of extensive investigations, including multiple randomized controlled trials. The proper treatment of patients with carotid bifurcation disease is of major interest to vascular surgeons and other vascular specialists. In 2011, the Society for Vascular Surgery published guidelines for treatment of carotid artery disease. At the time, several randomized trials, comparing carotid endarterectomy (CEA) and carotid artery stenting (CAS), were published.

Invited commentary
In the current issue of the Journal of Vascular Surgery, Solomon et  al1 described the perioperative outcome after transfemoral carotid artery stenting (CAS) stratified by preprocedural symptomatic status with a special emphasis on outcome in patients with asymptomatic and formerly symptomatic (>180  days). In s previously reported study, the outcome after carotid endarterectomy (CEA) that stroke morbidity and mortality after CEA depended on carefully defined symptomatic status. The authors applied the same methodology to their present study on CAS using data from the National Surgical Quality Improvement Program in 9807 patients.

Carotid double jeopardy
Tandem carotid artery stenoses, one at the carotid bifurcation and one more proximal in the common carotid artery or innominate artery, are rare. I suspect that their incidence may increase with the ubiquitous availability of computed tomography angiography. Treatment of these lesions with a hybrid approach with stent placement or angioplasty of the proximal lesion and open endarterectomy of the carotid bifurcation was first reported in 1996.1 A prior meta-analysis showed excellent results.2 More recent reports have urged caution because the stroke and death rate were almost reaching unacceptable levels.
