Medicare Patients with Symptomatic Carotid Disease Requiring Carotid Revascularization Are Likely to Have Delayed Access: An Analysis of a Multicenter Surgical Data
The Society for Vascular Surgery clinical practice guidelines for management of extracranial cerebrovascular disease recommends carotid revascularization as soon as the patient is neurologically stable after 48  hours and before 14 days of symptom onset in patients with recent stroke. In the United States, insurance status has been demonstrated to play a role in wait times for surgical care. There are no large-scale studies that assess the effect of patient primary insurance status on time to surgery in patients with symptomatic carotid disease.

Using a CT Angiography Artificial Intelligence Carotid Plaque Morphology Model to Predict Stroke
The gold standard for determining carotid artery stenosis is to calculate stenosis using the North American Symptomatic Carotid Endarterectomy Trial criteria and ultimately plan for medical vs surgical management based on percent stenosis and symptomatic status. Few studies have assessed plaque morphology as an additive tool for stroke prediction. Our study uses an artificial intelligence software in conjunction with a patient's computed tomography (CT) scan of the neck to create a three-dimensional (3D) model of the carotid artery and assess plaque morphology including calcification, intraplaque hemorrhage, matrix, and pe...

Sex differences in Outcome after Carotid Revascularization in Symptomatic and Asymptomatic Carotid Artery Stenosis
Sex differences regarding the safety and efficacy of carotid revascularization in carotid artery stenosis have been addressed in several studies with conflicting results. Moreover, women are underrepresented in clinical trials leading to limited conclusions regarding the safety and efficacy of acute stroke treatments.

Postoperative Disability and One-Year Outcomes for Patients Suffering a Stroke after Carotid Endarterectomy
Although post-carotid endarterectomy (CEA) strokes are rare, they can be devastating. The degree of disability that patients develop after such events and its effects on long-term outcomes are unclear. Our goal was to assess the extent of postoperative disability in patients suffering strokes after CEA and evaluate its association with long-term outcomes.

Transcarotid Artery Revascularization Sustains Its Advantage Over Transfemoral Carotid Artery Stenting in the Longer Term: Propensity Score-Matched Three-Year Outcomes
The superiority of transcarotid artery revascularization (TCAR) over transfemoral carotid artery stenting (TFCAS) has been established in the perioperative period with a lower risk of stroke and death. However, it remains unclear whether TCAR will sustain its advantage in the longer term. We evaluated the 3-year outcomes for death, transient ischemic attack (TIA), stroke, and myocardial infarction (MI) after TCAR and TFCAS.

Vertebral Stump Syndrome and Recurrent Embolic Stroke in Posterior Circulation: Surgical Treatment of Complex Cases
Vertebral artery stump syndrome refers to the migration of thrombi from an occluded vertebral artery origin into the brain, causing posterior circulation strokes. Treatment of vertebral stump syndrome has been debated, and no definite treatment has been established.

Long-Term Outcome in Internal Carotid Endarterectomy in Asymptomatic Octogenarians
Internal carotid endarterectomy for asymptomatic octogenarian patients has been the subject of high controversy. The goal of this ongoing retrospective investigation is to determine the general risk factors and predictors of the perioperative and long-term outcomes to determine how far the age limit might be extended and which indicators might be of relevance for stroke-free survival.

Association of Beta Blockers and Calcium Channel Blockers with Arrhythmia After Carotid Intervention
Postoperative arrhythmia has a reported incidence of 2% to 4%, which can increase patients ’ risk of developing stroke or myocardial infarction; however, insufficient data are available regarding the prognostic implications and management of arrhythmia after carotid intervention. The aim of this study was to identify the factors associated with arrhythmia after carotid intervention usin g data collected as a part of the Society for Vascular Surgery Vascular Quality Initiative.

Carotid Artery Coiling Associated with Ipsilateral Ischemic Stroke Treated by Mechanical Thrombectomy
We have presented a case of asymptomatic internal carotid artery coiling resulting in a devastating ipsilateral ischemic stroke that was treated by intracranial mechanical thrombectomy.

Endovascular Treatment of Acute Ischemic Stroke —The Results of the METRICS Study (Mechanical Thrombectomy Quality Indicators Study in Czech Stroke Centers)
Rigorous and regular evaluation of defined quality indicators is crucial for further improvement of both technical and clinical results after mechanical thrombectomy (MT) for acute ischemic stroke. With the recent international multi-society consensus quality indicators, we aimed to assess whether centers in the Czech Republic performing endovascular treatment of acute ischemic stroke have met these recommendations.

Embolic Protection Devices for Carotid Artery Stenting: A Network Meta-Analysis
Carotid artery stenting (CAS) is an alternative treatment option for patients at high risk of carotid endarterectomy but has been correlated with an increased risk of distal embolization and periprocedural stroke despite the use of adjunctive embolic protection devices (EPDs). The present study compared three types of EPDs and their intra- and periprocedural-related complications.

The Challenge of Preventing Carotid Artery Stroke in Properly Medically Managed Patients: Lessons From SPACE-2
The SPACE-2 trial led by Dr. Tilman Reiff(1) from Heidelberg University was a multi-center, randomized, controlled trial including 36 centers in Austria, Germany and Switzerland. It aimed to directly compare the benefit of intervention versus best medical therapy (BMT) in high-grade asymptomatic carotid artery stenosis patients. Interventions considered were carotid endarterectomy (CEA) and carotid artery stenting (CAS). Participants were between the ages of 50 and 85 years with at least 70% internal carotid artery stenosis.
