Atrial fibrillation (AF) is the most common arrhythmia and is associated with an increased risk of ischemic stroke. Oral anticoagulation (OAC) has been shown to reduce the risk of ischemic stroke in patients with AF. Previous studies have demonstrated that both females and Black, Hispanic, and Asian groups are less likely to be prescribed OAC.

Po-03-135 left atrial appendage occlusion versus standard of care in patients with atrial fibrillation and a prior thrombo-embolic event despite oral anticoagulant therapy: a propensity score matched comparison
Patients with atrial fibrillation (AF) that suffer from ischemic stroke despite oral anticoagulant (OAC) therapy have a very high risk of recurrent stroke and better prevention strategies are needed. Left atrial appendage occlusion (LAAO) is a promising secondary prevention strategy that may provide mechanical protection. However, evidence showing effectiveness of LAAO in this population is scarce and the current international guidelines only consider LAAO in patients with a contra-indication for OAC.

Patients with cryptogenic stroke (CS) undergo insertable cardiac monitor (ICM) screen for occult atrial fibrillation (AF) as a possible ischemic stroke etiology. Continuous monitoring with ICM was shown to be superior to intermittent cardiac monitoring in the detection of AF in the CRYSTAL-AF Trial. However, there is limited real-world data about the rate of AF detection using ICM in patients with cryptogenic stroke.

Atrial fibrillation (AF) is a common type of sustained arrhythmia worldwide. Asymptomatic AF, which occurs frequently, is associated with an increased incidence of ischemic stroke, heart failure, and mortality. A large number of patients with paroxysmal atrial fibrillation (PAF) remain undiagnosed due to the absence of electrocardiographic evidence of AF (AF-ECGs). If PAF could be predicted, targeted screening could improve early detection and treatment of this condition.

Po-713-05 ischemic stroke after prophylaxis with left atrial appendage closure vs non-warfarin oral anticoagulants: a “real-world” multicenter comparison of stroke severity
In randomized trials, strokes occurring in patients who have received left atrial appendage closure (LAAC) devices have been less severe than in patients receiving warfarin prophylaxis - an observation thought related to warfarin ’s higher rate of hemorrhagic stroke (which are known to have worse outcomes). Similar stroke rates have been described with LAAC vs NOACs (non-warfarin oral anticoagulants), but the low hemorrhagic stroke rate of NOACs raises the question of stroke severity between these two therapies.
