Hypertensive response to exercise in adult patients with repaired aortic coarctation
Conclusions A hypertensive response to exercise was present in nearly half of the patients in this large, prospective cohort of adults with repaired CoA. Risk factors for increased peak exercise SBP were male sex, absence of a bicuspid aortic valve and elevated resting SBP. Increased peak exercise SBP independently predicted hypertension at follow-up. These results support close follow-up of patients with a hypertensive response to exercise to ensure timely diagnosis and treatment of future hypertension.

Sex and gender matter in cardiovascular disease and beyond
Sex and gender are fundamental drivers of virtually all major causes of death and disease, while gender equality has been shown to improve the health of both women and men at the population level. The term ‘sex’ is generally used to describe biological characteristics, while ‘gender’ is used to address social constructs. Sex and gender are intertwined and interconnect with other key drivers of health, such as age, socioeconomic position, race and ethnicity. Over the past decade, many clinically meaningful sex differences in several aspects of cardiovascular disease (CVD) have been uncovered. Althoug...

Prediction of incident atrial fibrillation in community-based electronic health records: a systematic review with meta-analysis
Conclusions Models externally validated for prediction of incident AF in community-based EHR demonstrate moderate predictive ability and high risk of bias. Novel methods may provide stronger discriminative performance. Systematic review registration PROSPERO CRD42021245093.

Evaluation of antithrombotic use and COVID-19 outcomes in a nationwide atrial fibrillation cohort
Conclusions Pre-existing AT use may be associated with lower odds of COVID-19 death and, while not evidence of causality, provides further incentive to improve AT coverage for eligible individuals with AF.

Association of carbohydrate and saturated fat intake with cardiovascular disease and mortality in Australian women
Conclusions In middle-aged Australian women, moderate carbohydrate intake (41.0%–44.3% of TEI) was associated with the lowest risk of CVD, without an effect on total mortality. Increasing saturated fat intake was not associated with CVD or mortality and instead correlated with lower rates of diabetes, hypertension and obesity.

Cardiac myxomas: clinical presentation, diagnosis and management
Cardiac myxomas (CM) are the most common type of primary cardiac tumours in adults, which have an approximate incidence of up to 0.2% in some autopsy series. The purpose of this review is to summarise the literature on CM, including clinical presentation, differential diagnosis, work-up including imaging modalities and histopathology, management, and prognosis. CM are benign neoplasms developed from multipotent mesenchyme and usually present as an undifferentiated atrial mass. They are typically pedunculated and attached at the fossa ovalis, on the left side of the atrial septum. Potentially life-threatening, the presence ...

Correspondence on 'Omega-3 supplementation and cardiovascular disease: formulation-based systematic review and meta-analysis with trial sequential analysis' by Rizos et al
In their review and meta-analysis, Rizos et al1 reported that omega-3 supplementation at low and higher dosages showed no or weak associations with cardiovascular disease (CVD) outcomes, respectively. More recent reviews also showed protective activity of omega-3 against CVD outcomes. For instance, omega-3 (2–4 g/day) in patients with high or very high triglyceride (TG) levels decreases significantly high-sensitivity C reactive protein (hsCRP), lipoprotein-associated phospholipase A2 (Lp-PLA2), and oxidised low density lipoprotein (LDL)-cholesterol (oxLDL) levels, and the reduction of hsCRP is enhanced when co-admini...

Trends in the pharmacological management of atrial fibrillation in UK general practice 2008-2018
Conclusions There has been an increase in the proportion of patients with AF appropriately prescribed anticoagulants following National Institute for Health and Care Excellence and European Society of Cardiology guidelines, which correlates with improvements in mortality and stroke outcomes. Beta-blockers appear increasingly favoured over digoxin for rate control. There has been a steady decline in GP prescribing rates for rhythm control drugs, possibly related to concerns over efficacy and safety and increased availability of AF ablation.

Refined atrial fibrillation screening and cost-effectiveness in the German population
Conclusions In our middle-aged population cohort, we identified 0.2% new AF by single timepoint screening. There was a significant estimated risk of stroke and heart failure in these individuals. Cost-effectiveness for screening may be reached in individuals aged 65 years and older. The simple age cut-off is not improved by using Nt-proBNP as a biomarker to guide a screening programme.

Effectiveness and safety of oral anticoagulants in elderly patients with atrial fibrillation
Conclusion In this nationwide cohort study of patients ≥75 years initiating oral anticoagulation for AF, standard and reduced dose NOACs were associated with similar risks of stroke/SE as warfarin and lower or similar risks of bleeding. The NOACs seem to be a safe option also in elderly patients.

Atrial fibrillation and risk of incident heart failure with reduced versus preserved ejection fraction
Conclusions AF is associated with both HFrEF and HFpEF events, with no significant difference in the strength of association among these subtypes.

Association of physical activity trajectories with major cardiovascular diseases in elderly people
Conclusion Increasingly active trajectories of physical activity were associated with lower rates of cardiovascular diseases and overall mortality. Promoting at least 20 min/day of physical activity early in late life seems to provide the greatest cardiovascular benefits.

Smoking cessation, weight gain and risk of cardiovascular disease
Conclusions Postcessation weight gain did not attenuate the protective association between smoking cessation and risk of CVD.

Dengue virus infection induces inflammation and oxidative stress on the heart
Conclusions DENV-3 infection induced a marked cardiac dysfunction, which may be associated with inflammation, oxidative stress and electrophysiological changes in the heart. These findings provide new cardiac insights into the mechanisms involved in the pathogenesis triggered by DENV, contributing to the research of new therapeutic targets for clinical practice.
