Dual potential embolic source detected in left atrium during hyperacute stroke
Clinical introduction A woman in her late 60s with a medical history of hypertension presented to the emergency department with symptoms of a stroke which began 40 min prior. Brain MRI was performed (figure 1A). Hyperacute CT angiography of the head and neck did not find any atherosclerosis. Fixed dose combination therapies in primary cardiovascular disease prevention in different groups: an individual participant meta-analysis

Subannular repair in secondary mitral regurgitation with restricted leaflet motion during systole
Conclusions Our study demonstrates the safety and feasibility of standardised subannular repair to treat ventricular SMR (Carpentier type IIIb) in a multicentre setting. By addressing mitral leaflet tethering, papillary muscle relocation results in very satisfactory 1-year outcomes and has the potential to durably restore MV geometry; nevertheless, long-term FU is mandatory. Bradyarrhythmias in patients with atrial fibrillation
The increasing burden of atrial fibrillation (AF) and significant morbidity/mortality associated with this arrhythmia encourage screening it in clinical practice, as recommended in current guidelines. Primary prevention of cardiovascular disease in women with a Mediterranean diet: systematic review and meta-analysis
Conclusion This study supports a beneficial effect of the Mediterranean diet on primary prevention of CVD and death in women, and is an important step in enabling sex specific guidelines.

Elective ascending aortic aneurysm repair outcomes in a nationwide US cohort
Conclusions The overall rate of in-hospital death, stroke and MI is nearly 5% in patients undergoing elective ascending aortic aneurysm repair. Among several predictors, chronic weight loss is associated with the largest increase in the risk of poor outcomes. Higher hospital volume is associated with a lower in-hospital mortality, highlighting the importance to refer patients to high-volume centres while discussing the risks and benefits of proceeding with repair.
