Authors: Kwon SU, Kim JS Abstract Symptomatic cerebral atherosclerosis including intracranial atherosclerosis (ICAS) is associated with a high risk of recurrent stroke. Antithrombotic agents are the mainstay of therapy in these patients. Several studies have found anticoagulation (warfarin) to increase the risk of bleeding events and have an efficacy no better than that of aspirin. Intracranial Fibromuscular Dysplasia in a Middle-Aged Woman with Recurrent Ischemic Stroke (P4.388)
Conclusions: FMD is a rare disease and has very rarely been described in the intracranial arteries. This case illustrates the natural history and progression of FMD in this patient and elucidates the limited treatment options for preventing disease progression and secondary complications. FMD most predominantly affects younger to middle-aged females and if the disease process goes unrecognized it can have devastating consequences. Very late ischemic complications in flow-diverter stents: a retrospective analysis of a single-center series.

Clinical characteristics, management and 1-year outcomes of patients with acute coronary syndrome in Iran: the Iranian Project for Assessment of Coronary Events 2 (IPACE2)
Conclusions Our study showed that the composition of Iranian patients with ACS regarding the type of ACS is similar to that in developed European countries and is unlike that in developing countries of the Middle East and Africa. We found that our patients with ACS are treated with high levels of adherence to guideline-recommended in-hospital medications.

Safety of percutaneous coronary intervention in patients with acute ischemic stroke/transient ischemic attack and acute coronary syndrome
Conclusions The primary and secondary end points were not enhanced in patients undergoing PCI. Therefore PCI is safe in patients with stroke/TIA and concomitant ACS.

Dual antiplatelet therapy in stroke and ICAS: Subgroup analysis of CHANCE
Conclusions: The results indicated higher rate of recurrent stroke in minor stroke or high-risk TIA patients with ICAS than in those without. However, there was no significant difference in the response to the 2 antiplatelet therapies between patients with and without ICAS in the CHANCE trial. Classification of evidence: This study provides Class II evidence that for patients with acute minor stroke or TIA with and without ICAS identified by MRA, clopidogrel plus aspirin is not significantly different than aspirin alone in preventing recurrent stroke.

Acute Myocardial Infarction and Acute Stroke: Between a Rock and a Hard Place
A 53year-old woman developed severe chest pain, breathlessness and palpitations whilst being told some very devastating news. As the symptoms persisted for more than 15minutes, electrocardiography was undertaken in the community confirming an anterolateral STEMI. Following the electrocardiographic findings the patient was loaded orally in the community with aspirin 300mg and clopidogrel 600mg and was urgently transferred via the community activation primary PCI route to our institution for emergency coronary angiography.
