Risk of stroke following antivenom use after venomous snakebite: correspondence
We read with great interest the epidemiological study by Hunget al.1 on the risk of stroke with antivenom use after venomous snakebite in Taiwan. According to the national medical claims data, the authors provide us with useful information and disclose that venomous snakebite is associated with an increased risk of hemorrhagic stroke after the use of antivenom after using the matching propensity score in relevant measurable covariates. We strongly agree with the comments by Yehet al.2 on the unmeasured confounders associated with stroke in the present study. Furthermore, we are concerned that other residual confounders rel...

Effects of heavy metals in acute ischemic stroke patients: A cross-sectional study
Cerebrovascular disease is the second commonest cause of mortality globally and among the commonest causes of disability. However, research executed to probe the heavy metal exposure–stroke incidence relationship is scarce. Accordingly, we executed our study to probe the relationship of heavy metal concentrations (ie, concentrations of lead [Pb], mercury [Hg], cadmium [Cd], and arsenic) in the serum and urine of acute ischemic stroke (AIS) patients with several patient variables. For enrollment, we chose patients who had a first AIS within 7 days after the onset of a stroke. Thus, 33 newly diagnosed patients with AI...

Hypothyroidism risk associated with rheumatoid arthritis: A population-based retrospective cohort study
Studies on the thyroid disease risk in patients with rheumatoid arthritis (RA) associated with comorbidities are limited. This population-based retrospective cohort study investigated the hypothyroidism risk in patients with RA and the role of comorbidities. We used Taiwan National Health Insurance Research Database to identify 16,714 RA patients newly diagnosed in 2000 to 2008 and 66,856 control persons without RA, frequency matched by sex, age, and index year. Incidence and the RA group to controls hazard ratio of hypothyroidism were estimated. The hypothyroidism incidence was 1.74-fold higher in the RA group than ...

Prognostic indicators for the onset of ischaemic versus haemorrhagic stroke in stable coronary artery disease
The incidence of stroke may be increased in patients with coronary artery disease (CAD). We aimed to investigate the specific risk factors for the development of ischaemic and haemorrhagic stroke in stable CAD patients. Patients with stable CAD were prospectively enrolled for future cardiovascular events in Taiwan. All the patients had received coronary interventions and were stable for least 1 month before enrolment. The incidence of ischaemic stroke was identified and confirmed by telephone and hospital records. Baseline characteristics, including demographic data, lipid profiles, medications, and biomarkers for pot...

Renal dysfunction is associated with lower odds of home discharge for patients with stroke
CONCLUSION: The odds of home discharge for stroke patients decreased with a significant independent graded association with declining eGFR levels. Renal function could predict home discharge after stroke.PMID:34351833 | DOI:10.1080/00325481.2021.1964198

Telmisartan use and risk of dementia in type 2 diabetes patients with hypertension: A population-based cohort study
by Chi-Hung Liu, Pi-Shan Sung, Yan-Rong Li, Wen-Kuan Huang, Tay-Wey Lee, Chin-Chang Huang, Tsong-Hai Lee, Tien-Hsing Chen, Yi-Chia Wei BackgroundAngiotensin receptor blockers (ARBs) may have protective effects against dementia occurrence in patients with hypertension (HTN). However, whether telmisartan, an ARB with peroxisome proliferator-activated receptor γ (PPAR-γ)–modulating effects, has additional benefits compared to other ARBs remains unclear. Methods and findingsBetween 1997 and 2013, 2,166,944 type 2 diabetes mellitus (T2DM) patients were identified from the National Health Insurance Research Database of Taiwa...

Longer length of post-acute care stay causes greater functional improvements in poststroke patients
Post-acute care (PAC) is a type of transitional care for poststroke patients after the acute medical stage; it offers a relatively intensive rehabilitative program. Under Taiwan's National Health Insurance guidelines, the only patients who can transfer to PAC institutions are those who have had an acute stroke in the previous month, are in a relatively stable medical condition, and have the potential for improvement after aggressive rehabilitation. Poststroke patients receive physical, occupational, and speech therapy in PAC facility. However, few studies have evaluated the effects of PAC in poststroke patients since PAC's...

Primary prevention of cardiovascular disease events with renin-angiotensin system blockade in autosomal dominant polycystic kidney disease dialysis patients: A nationwide cohort study
Although renin-angiotensin system (RAS) blockade has been shown to reduce cardiovascular disease (CVD) in the general population and high-risk subjects, their protective effect in autosomal dominant polycystic kidney disease (ADPKD) patients under dialysis was still unknown. By using the database from 1995 to 2008 Taiwan National Health Insurance Research Database (Registry for Catastrophic Illnesses), we included 387 ADPKD patients who received dialysis therapy, aged ≥ 18 year-old, and with no evidence of CVD events in 1997 and 1998. We utilized Cox proportional hazards regression analysis and propensity score mat...
