Ann Med Surg (Lond). 2023 May 3;85(6):3026-3030. doi: 10.1097/MS9.0000000000000712. eCollection 2023 Jun.ABSTRACTMitochondrial encephalomyopathy, lactic acidosis, and stroke-like syndrome (MELAS) is a rare neurodegenerative inherited disorder that is characterized by stroke-like episodes, seizures, endocrine, and multiple system involvement. It is important to consider it as a differential diagnosis in a young patient with stroke-like episodes as it is progressive and has multiple complications.CASE PRESENTATION: A 28-year-old male presented with slurring of speech and drowsiness for 7 h. Credit: UNHCRBy Simone GalimbertiKATHMANDU, Nepal, Apr 29 2022 (IPS) The recently disseminated Zero Draft Ministerial Declaration of the High-Level Political Forum on Sustainable Development (HLPF)– the main UN event to track the member states’ progress to achieve the Agenda 2030 slated to be held in the first half of July– is a disappointment. For all its comprehensiveness, the document neglected to mention one of the most...

Estimation of mean population salt intakes using spot urine samples and associations with body mass index, hypertension, raised blood sugar and hypercholesterolemia: Findings from STEPS Survey 2019, Nepal
by Saroj Bhattarai, Bihungum Bista, Binod Kumar Yadav, Pradip Gynawali, Anil Poudyal, Anjani Kumar Jha, Meghnath Dhimal BackgroundHigh dietary salt intake is recognized as a risk factor for several non-communicable diseases (NCDs), in particular cardiovascular diseases (CVDs), including heart attack and stroke. Accurate measurement of population level salt intake is essential for setting targeted goals and plans for salt reduction strategies. We used a spot urine sample to estimate the mean population salt intake in Nepal and evaluated the association of salt intake with excess weight, hypertension, raised blood sugar and ...

Low-cost alternatives for the management of acute ischemic stroke in low and middle-income countries
Ann Med Surg (Lond). 2021 Oct 21;72:102969. doi: 10.1016/j.amsu.2021.102969. eCollection 2021 Dec.ABSTRACTAcute ischemic stroke (AIS) patients arriving within a suitable time frame are treated with recanalization therapy i.e. intravenous thrombolysis (IVT) with alteplase and/or mechanical thrombectomy (MT). IVT with alteplase is indicated in AIS patients presenting within 4.5 hours of onset regardless of vascular territory involved. MT is indicated in AIS patients presenting within 24 hours of onset with large vessel occlusion in the anterior circulation. However, MT is ludicrously expensive and requires exorbitant setup, ...

Role of female community health volunteers in ischemic stroke prevention, identification, referral and rehabilitation in Nepal
Ann Med Surg (Lond). 2021 Oct 8;72:102893. doi: 10.1016/j.amsu.2021.102893. eCollection 2021 Dec.ABSTRACTFor the past three decades, female community health volunteers (FCHVs) have been at the forefront of Nepal's health map and have contributed significantly to its improving health indicators such as maternal mortality rate and infant mortality rate. Given the changing epidemiology of Nepal and the shift of burden from communicable to non-communicable diseases (NCDs), it is important to revitalize their role with the changing times. The prevalence of ischemic stroke in Nepal is on the rise. However, very few people make i...

An opportunity to improve Acute Ischemic Stroke care in the South Asian region through telestroke services
Ann Med Surg (Lond). 2021 Nov 25;72:103115. doi: 10.1016/j.amsu.2021.103115. eCollection 2021 Dec.ABSTRACTThe South Asian region accounts for more than 40% of the stroke burden of the developing world and is the highest contributor to global stroke mortality. Despite proven treatment guidelines, the limited number of neurologists, the number of dedicated stroke centers and the fact that most of these facilities are urban-centric renders poor access to thrombolysis in this region, especially in the rural areas. Studies have shown that thrombolysis using telestroke services are comparable to face-to-face thrombolysis. Telest...

Radioimaging and Demographic Profiles of Patients with Spontaneous Intracerebral Hemorrhage: A Need for the Pragmatic Referral System
Adv Exp Med Biol. 2021 Nov 14. doi: 10.1007/5584_2021_662. Online ahead of print.ABSTRACTReferring patients with severe medical conditions, like brain stroke, from small or rural hospitals to tertiary care centers is often overexploited leading to a kind of defensive medicine. The issue of a patient referral system remains unaddressed in Nepal. In this article, we investigated the legitimacy of referring patients with spontaneous intracranial hemorrhage (ICH) from country peripheral hospitals to the leading tertiary neurological center in Nepal. We found that 81 out of the 130 ICH patients reviewed in the study were referr...
