Orthostatic hypotension and cardiovascular risk.
Abstract Orthostatic hypotension (OH) is a cardinal sign of cardiovascular (CV) autonomic dysfunction as a result of autonomic nervous system failure to control the postural hemodynamic homeostasis. The proportion of individuals with OH increases with aging and chronic conditions, such as neurodegenerative diseases, hypertension, heart failure, diabetes, renal dysfunction, autoimmune diseases and cancer. In those over 70 years, more than 20% can be affected. It is now increasingly recognized that a direct relationship exists between OH and each step of the CV disease continuum, eventually leading to end-stage hear...

Causes of death and morbidity in patients with atrial fibrillation after left atrial appendage occlusion.
CONCLUSION: The majority of deaths are not related to stroke in patients with AF after the LAAO procedure. Mortality in first 2 years following the procedure was predominantly from non-cardiovascular causes. Key words: atrial fibrillation, LAAO, left atrial appendage, stroke. Depressive symptoms and cardiovascular diseases in the adult Polish population. Results of the NATPOL2011 study.
CONCLUSIONS: Depressive symptoms were more prevalent in women compared to men, and they were significantly and inde-pendently associated with age and primary education level in both sexes, and with a history of stroke in women. PMID: 30406941 [PubMed - indexed for MEDLINE]

Statement by a Working Group conceived by the Polish National Consultants in Cardiology and Neurology addressing the use of implantable cardiac monitors in patients after ischaemic embolic stroke of undetermined source.
Abstract INTRODUCTION: Stroke remains one of the main causes of death and the most common cause of long-term disability in adults. Embolic strokes of undetermined source (ESUS) amount to a significant proportion of all ischaemic strokes. Detection of atrial fibrillation (AF) in this group of patients would allow for a major therapeutic decision to switch from antiplatelets to oral anticoagulants and therefore significantly reduce the risk of recurrence. STATE OF THE ART: Current technology allows long-term continuous ECG monitoring with different systems, including implantable cardiac monitors (ICM). However,...

Transthoracic echocardiography in the assessment of cardiogenic causes of ischaemic stroke.
CONCLUSIONS: 1. Transthoracic echocardiography performed routinely in all AIS patients affects the treatment in a very low percentage of cases. 2. The group that could benefit the most from TTE examination includes people without established indications for chronic anticoagulant therapy, in particular patients after myocardial infarction and people with additional clinical symptoms. 3. In patients with AIS, the diagnostic sensitivity of TTE in the detection of PFO is low. Young people with a cryptogenic ischaemic stroke should undergo a transoesophageal assessment. Revisiting the risks of incident atrial fibrillation: a narrative review. Part 2.
Abstract Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia with increasing incidence worldwide. Much focus has been directed towards AF prevention, given the morbidity and mortality from stroke, heart failure, and dementia. There are a number of common conditions associated with the onset of AF including, but not limited to, increased alcohol consumption, body weight, exercise, and stress. To reduce the incidence of AF, public health campaigns and targeted patient interventions may be warranted to promote balanced alcohol intake, appropriate exercise, and stress management to prevent AF and ...

Non-vitamin K antagonist oral anticoagulants (NOACs) in the treatment of coronary and peripheral atherosclerosis. Expert Consensus.
Abstract Oral anticoagulants (OAC) are widely used for prevention of systemic thromboembolism, including the reduction of the risk of stroke in patients with atrial fibrillation (AF) and prosthetic heart valves. There is also an increasing population of patients who require not only OACs, but also double antiplatelet therapy (DAPT). A typical example is a patient with AF and stable coronary artery disease or acute coronary syndrome (ACS), treated by percutaneous coronary intervention (PCI). In recent years, with the introduction of NOACs, triple or dual therapy has become safer. Regardless of these indications for...

Ministernotomy or sternotomy in isolated aortic valve replacement? Early results.
Conclusions: Ministernotomy for AVR is a safe method and does not increase morbidity and mortality. It significantly reduces post-operative blood loss and shortens hospital stay. Ministernotomy can be successfully used as an alternative method to sternotomy. PMID: 30647743 [PubMed]

Left atrial appendage occlusion: consensus document of Association of Cardiovascular Interventions and Heart Rhythm Section of Polish Cardiac Society.
Abstract Left atrial appendage (LAA) occlusion devices have the potential to influence the clinical approach to stroke prevention in patients with atrial fibrillation. A number of percutaneous techniques have been proposed, including various intracardiac plugs and also external ligation. Several devices have been already used in Poland. One of them has been evaluated in randomised controlled trials compared with the current standard of care. Others are less well studied but quite commonly used in Eu-rope. It is anticipated that the use of LAA occlusion technologies in clinical practice will expand. This Consensus ...

Differences in knowledge among patients with atrial fibrillation receiving non-vitamin K antagonist oral anticoagulants and vitamin K antagonists.
CONCLUSIONS: The knowledge of arrhythmia and anticoagulation is better regarding the safety issues among subjects on NOACs compared with those on VKAs. Irrespective of the type of oral anticoagulation therapy, education of AF patients should be improved. PMID: 29528483 [PubMed - indexed for MEDLINE]

Applications of low-cost 3D printing in left atrial appendage closure using epicardial approaches - initial clinical experience.
Conclusions: Left atrial appendage 3D model is a useful tool in preoperative planning of a left atrial appendage occlusion using epicardial approaches with thoracoscopic or percutaneous access using LARIAT device. The quality of low-cost 3D printed LAA model is sufficient in planning minimally invasive procedure. PMID: 30069196 [PubMed]
