The importance of atrial fibrillation and selected echocardiographic parameters for the effectiveness and safety of thrombolytic therapy in patients with stroke
ConclusionsThe presence of atrial fibrillation worsens the patient's prognosis in terms of the functional status and survival during the acute period of stroke in patients treated with intravenous thrombolysis.Higher NIHSS and CHA2DS2VASc scores and reduced EF in patients with stroke treated with thrombolysis are the predictors of unfavorable short-term prognosis.

Malignant middle cerebral artery (MCA) infarction in people over 85 years old – Diagnosis, management and risk factors
ConclusionThe key risk factor in this group is the atrial fibrillation. The elderly patients require an intensive monitoring of the health condition by reference to brain stroke risk factors, especially atrial fibrillation.

Differences in knowledge among patients with atrial fibrillation receiving NOACs and vitamin K antagonists.
CONCLUSIONS: The knowledge of arrhythmia and anticoagulation is better regarding the safety issues among subjects on NOACs compared with VKA. Irrespective of the type of OAC therapy, education of AF patients should be improved. PMID: 29528483 [PubMed - as supplied by publisher]

Left atrial appendage occlusion: consensus document of Association of Cardiovascular Interventions and Heart Rhythm Section of Polish Cardiac Society.
Abstract Left atrial appendage (LAA) occlusion devices have the potential to influence the clinical approach to stroke prevention in patients with atrial fibrillation. A number of percutaneous techniques have been proposed, including various intracardiac plugs and also external ligation. Several devices have been already used in Poland. One of them has been evaluated in randomised controlled trials compared with the current standard of care. Others are less well studied but quite commonly used in Europe. It is anticipated that the use of LAA occlusion technologies in clinical practice will expand. This Consensus D...

Dual antiplatelet therapy is safe and efficient after left atrial appendage closure.
CONCLUSIONS: LAAC followed by DAPT seems to be a safe and efficient alternative for stroke prevention in patients with NVAF and contraindications to anticoagulation therapy. This strategy may provide a significant reduction of events such as stroke and bleeding versus the score-predicted rate. PMID: 29350390 [PubMed - as supplied by publisher]

The evaluation of the recommended prevention of thrombosis in hospitalized patients with atrial fibrillation and high thromboembolism risk.
ConclusionsOral anticoagulants was indicated on discharge for 2/3 of patients with atrial fibrillation and high risk of stroke, and more often in patients with low risk of bleeding events. An increase in the number of oral anticoagulants indications has been observed in the recent 9 years. The factors which led to the lack of oral anticoagulants indications among patients with a high stroke risk were: hospitalization in years 2004-2006, high risk of bleeding, vascular disease, age≥80 years, paroxysmal atrial fibrillation and previous bleeding. Importance rating of risk factors of ischemic stroke in patients over 85 years old in the polish population
Conclusion The course of brain stroke in the Polish population is more severe in patients over 85 years old than in younger ones. The key risk factor in this group is atrial fibrillation.

Usefulness of N-terminal-pro B-type natriuretic peptide as a heart failure marker in patients undergoing the percutaneous left atrial appendage occlusion.
CONCLUSIONS: When diagnosing HF in atrial fibrillation patients, the higher cut-off value of NT-proBNP should be used. NT-proBNP remains an appropriate diagnostic marker of HF in patients after LAAO. PMID: 28980292 [PubMed - as supplied by publisher]

Screening For Asymptomatic Atrial Fibrillation: Its Effectiveness And Cost-Effectiveness Of Repeated Ecg In Poland
Atrial fibrillation (AF) is the most common arrhythmia in humans. AF increases the risk of ischemic stroke up to 5-fold. Fortunately, this is highly preventable with appropriate oral anticoagulant (OAC) therapy. However, many patients with AF, despite the presence of risk factors, are not taking OACs, because they are not aware of the occurrence of arrhythmia prior to stroke. Therefore, it seems that the screening for AF in a broad population aimed at the earliest possible detection of asymptomatic arrhythmias, and the implementation of OAC treatment when necessary, could prevent many strokes.

Antiplatelet and anticoagulant therapy in elderly people with type 2 diabetes mellitus in Poland (based on the PolSenior Study).
CONCLUSIONS: Cardiovascular pharmacological prevention in the elderly with T2DM in Poland seems to be unsatisfactory. Educational programmes concerning current recommendations for pharmacological cardiovascular prevention should be developed among general practitioners. PMID: 28883841 [PubMed]

Atrial fibrillation and stroke as initial manifestations of painless type A aortic dissection
In conclusion, apart from imaging the aortic arch by computed tomography in acute stroke patients, the electrocardiogram may be indicative for aortic dissection if it shows signs for myocardial ischemia in previously healthy patients.
