Outcomes of Head and Neck Microvascular Reconstruction in Hypercoagulable Patients
Conclusion Microvascular head and neck reconstruction can be successfully performed in hypercoagulable subjects. However, patient-related complications remain a concern in these subjects. Overview of Evidence on Emergency Carotid Stenting in Acute Ischemic Stroke due to Tandem Occlusions: A Systematic Review and Meta-Analysis
Introduction - The combination of cervical internal carotid artery (ICA) occlusion with synchronous intracranial thrombus represents 10-20% of all acute ischemic strokes (AIS), and such tandem occlusions are strongly associated with poor outcome.

Symptomatic Carotid Plaques Show Decreased (18)F-Fluorodeoxyglucose (FDG) Uptake on Positron Emission Tomography (PET) After 3 Months Follow-up from Last Symptom
Introduction: The risk of stroke is high shortly after the index symptoms in patients with carotid disease. This indicates that unstable carotid plaques stabilizes over time, which may be due to changes in composition and inflammatory activity. Inflammation is thought to be one of the leading causes in atherosclerotic plaques destabilizing leading to thrombosis. With positron emission tomography (PET) previous studies have shown increased uptake of the glucose analogue tracer [18F]-fluorodeoxyglucose (FDG) in symptomatic compared to asymptomatic carotid plaques.

Incidence of major adverse cardiovascular events among patients with provoked and unprovoked venous thromboembolism: Findings from the Registro Informatizado de Enfermedad Tromboembólica Registry
ConclusionsAmong patients with VTE, provoked cases, specifically those with cancer-associated VTE, have an increased risk of major arterial events.

Haemodynamics of Different Configurations of a Left Subclavian Artery Stent Graft for Thoracic Endovascular Aortic Repair.
CONCLUSION: Arterial haemodynamic characteristics are affected by LSA branched stent graft configurations, with pressure drops and energy losses likely to be clinically insignificant. The length of the stent graft protrusion into the aortic lumen generated the largest haemodynamic variations in the aortic system. Protrusions up to 5 mm have smaller risk of potential thrombus generation. Conversely, larger protrusions into the aortic lumen showed more disturbed haemodynamics, suggesting a greater risk of potential thrombus formation, which may be clinically important over time. LEA 24. Synthetic Marijuana and Acute Lower Limb Thromboembolism and Ischemia —A Case Report
Synthetic marijuana use has been reported in the last years as a possible causative factor of different cardiovascular events, including myocardial infarction, ischemic stroke, intracranial bleeding, and cerebral vasospasm. One case of aortic thrombosis was also reported, but that was in a patient using cocaine and synthetic marijuana together. A case of lower limb thromboembolism and synthetic marijuana use has not been reported to date. Intoxication, material impurity, blood vessel reactivity, and chemical interaction with other drugs have been proposed as possible mechanisms of these events.

Impact of the physical activity on the phenotype of circulating monocytes and the biological characteristics of the carotid plaque
Atherosclerosis is accompanied by a low-grade inflammation which mobilizes the classical, intermediate and non-classical circulating monocytes. It was shown that an accumulation of leucocytes supports the development of intraplaque hemorrhage (IPH), making the plaque vulnerable, exposed to rupture and thrombosis. Moreover, the elevation of the rate of intermediate monocytes is associated with an increase in cardiovascular mortality and ischemic stroke. The benefit of endarterectomy (CE) remains discussed in the patients without stroke.

Carotid artery stenting in the elderly. Are there differences between open and closed cell stents?
CONCLUSION: Our data show CAS as a safe procedure also for people older than 75 years in terms of perioperative and long term complications and cerebral events without any significant difference between the different type of stent. Further studies are requested to better clarify its role in symptomatic patients. KEY WORDS: Carotid stenting, Carotid treatment, Old people. PMID: 31182697 [PubMed - in process]

Considerations Beyond Stenosis for Carotid Endarterectomy in Treating Free-Floating Thrombus of the Carotid Artery
Free-floating thrombus (FFT) of the carotid artery is an uncommon condition that can present with neurologic symptoms, often in the setting of ischemic stroke. The literature pertaining to the incidence and optimal treatment of this condition is limited. Herein we report our contemporary experience with FFT across a range of degrees of carotid stenosis.

Cerebral venous thrombosis in adults.
Abstract Thromboses of cerebral veins and/or sinuses (CVT) encompass a clinically rather heterogenous, relatively rare entity, which is nonetheless an important cause of stroke in the young. While until the mid of the last century the prognosis of CVT was thought to be poor or even fatal in the majority of cases, the advance and wider use of modern imaging technology since the early 1980s, together with the results of registries and population-based studies, revealed that CVT is more frequent and less life-threatening than previously assumed. However, because headache is frequently the only symptom, many oligosym...

Catheter-directed thrombolysis versus suction thrombectomy in the management of acute pulmonary embolism
ConclusionsCDT seems to have similar outcomes with ST in the management of acute PE, although larger, more homogenous data are needed. In our experience, ST should be viewed as a complementary alternative for patients with contraindication for thrombolytics or severely compromised hemodynamic profile and can yield good outcomes in an otherwise highly morbid population.

Invited commentary
Since current guidelines based on randomized clinical trials1-6 have established degree of carotid stenosis as the primary surrogate for stroke risk and indication for carotid endarterectomy or stenting, accurate assessment of the degree of carotid stenosis has been the traditional focus of carotid imaging.7 However, the degree of carotid stenosis is an inadequate assessment of stroke risk, and other key factors for determining carotid plaque vulnerability include intraplaque hemorrhage, plaque rupture, and luminal thrombus.

Persistently low inferior vena cava filter retrieval rates in a population-based cohort
ConclusionsIVC filter placements, after a substantial increase between 2004 and 2010, have been declining since 2010. Retrieval rates in the general population are steadily improving but continue to lag behind those described in center-specific literature. Increased age and Medicare as the primary payer are the strongest risk factors for lack of filter retrieval. Widespread improvements on a national scale are needed to improve the appropriateness of filter placements and to enhance filter retrieval rates.
