P83 The relationship of functional outcome after mechanical thrombectomy and serum glucose is partially mediated by edema formation
ConclusionEdema formation explains 15% of the total adverse effects of high blood glucose levels. Major pathways might include other direct effects of high blood glucose, such as altered vessel wall and thrombus characteristics and higher thrombogenicity of the cerebral blood flow.ReferencesThorén, M., et al. P89 "Not only Carotid: two unusual cases of treatment of steno-obstructions
ConclusionsThe persistence of fetal anastomosis between carotid and basilar artery would represent a posterior cerebral circulation risk for ischemia and embolic infarction; in these cases the menagement of carotid steno-obstructions is challenging and requires special considerations.ReferencesJin X, Sun L, Feng Z, Li X, et. al. Cerebral venous thrombosis after COVID-19 vaccination: the role for endovascular treatment
Cerebral venous thrombosis (CVT) is a unique cause of stroke. Population-based studies demonstrate an incidence of clinically significant CVT of 1.3–1.5 per 100 000 per year.1 2 This may be an underestimated incidence, as these studies were not based on review of imaging and the often benign nature of this entity may lead to underdiagnoses. In the JNIS paper published by Cleaver et al,3 the authors describe their experience with endovascular treatment (EVT) in patients with cerebral venous sinus thrombosis in patients following COVID-19 vaccination. CVT-related stroke primarily affects young adult and middle-aged pat...

O-012 Peri-procedural complications, clinical outcomes and recurrence/retreatment rates after endovascular versus surgical treatment of intracranial aneurysms: a real-world assessment from the NVQI-QOD cerebral aneurysm registry
ConclusionEndovascular treatment of intracranial aneurysms was associated with better overall clinical outcomes than surgical clipping with improved disability free survival and better disposition at discharge or follow-up, across both ruptured and unruptured aneurysms. Although intraoperative complications were higher for ruptured aneurysms treated with endovascular treatment, there was no difference in unruptured aneurysms versus surgical clipping. Post-operative complications were lower for the endovascular cohort regardless of presentation. Overall, these data support ongoing primary adoption of endovascular treatment ...

O-052 The pennsylvania post-market multicenter experience with flow re-direction endoluminal device (FRED)
ConclusionThe results of the early post-market experience with the FRED device show reasonable safety and adequate aneurysmal occlusion rates comparable to other flow diverters. O-061 Predictors of failed mechanical thrombectomy for large vessel occlusion: insights from the stroke thrombectomy and aneurysm registry (STAR)
ConclusionIn one of the largest studies to evaluate factors associated with failure MT in real world practice, we demonstrate that MTF is associated with significantly more complications and worse outcome. O-066 Actin imparts thrombolysis resistance in acute ischemic stroke clots - gelsolin accelerates TPA mediated thrombolysis

P-001 Evaluation of cerebrovascular flow parameters relative to guide catheter position during acute ischemic stroke treatment
ConclusionDramatically reduced MCA flow can significantly reduce downstream migration of thrombus during thrombectomy. This study demonstrates that Zoom 88 positioned distal to the PComm achieved similar rates of flow control as balloon guide catheters inflated in the proximal ICA. In all cases, flow compensation within the CoW affected the outflow rates of the ipsilateral and contralateral ACA branches, posterior cerebral arteries, and contralateral MCA.Abstract P-001 Figure 1Disclosures H. P-031 End artery location of thrombus predicts poor neurological outcome despite successful thrombectomy for anterior circulation stroke
ConclusionsDespite similar vascular territories, concurrent occlusion of the MCA and ACA segments results in worse clinical outcomes compared to more proximal ICA occlusion. The likely explanation for fast progression of infarct in the MCA+ACA group is the higher likelihood of end-artery involvement and lower contribution of collateral flow. Reperfusion of both territories; however, does add to the risk of symptomatic post-procedural hemorrhage.Disclosures S. Eshraghi: None. S. Saperian: None. A. Alawieh: None. B. Howard: None. J. P-032 Underlying intracranial atherosclerotic disease is associated with worse outcomes in acute large vessel occlusion undergoing endovascular therapy
ConclusionUnderlying ICAD is associated with a 2.7-fold increase in the odds of mortality and a 4.5-fold (1/0.22) increase in unfavorable functional outcome at 90 days in patients with LVO undergoing traditional MT. Further research is warranted to understand factors associated with poor outcomes and investigate alternative interventional approaches and medical management in this high-risk population.Disclosures S.
