Management of post-stroke mood and emotional disturbances.
Authors: Kim JS Abstract INTRODUCTION: Mood and emotional disturbances are common in stroke patients. Out of diverse post-stroke emotional disturbances, depression, anxiety, emotional incontinence, anger proneness, and fatigue are frequent and important symptoms. These symptoms are distressing for both the patients and their caregivers, and negatively influence the patient's quality of life. The emotional symptoms are not apparent and are therefore often neglected by neurologists. Their phenomenology, predicting factors, and pathophysiology have been under-studied, and are under-recognized. In addition, well-designed ...

Urinary Incontinence and Indwelling Urinary Catheters as Predictors of Death after New-Onset Stroke: A Report of the South London Stroke Register
To explore the relationship between indwelling urinary catheters (IUCs), urinary incontinence (UI), and death in the poststroke period and to determine when, after the neurological event, UI has the best ability to predict 1-year mortality.

How does sex affect the care dependency risk one year after stroke? A study based on claims data from a German health insurance fund.
DISCUSSION: It may be assumed that women have a higher risk of becoming care-dependent after stroke than men because they are older and suffer more often from geriatric conditions such as urinary incontinence at onset of stroke. Preventive strategies should therefore focus on geriatric conditions in order to reduce the post-stroke care dependency risk for women. PMID: 28330419 [PubMed - as supplied by publisher]

Urinary incontinence as a predictor of death after new‐onset stroke: a meta‐analysis
Conclusions Urinary incontinence indicates high risk of death after a new‐onset stroke. Validity of the analyses on adjusted models is limited by an obvious publication bias.

Glioproliferative Lesion of the Spinal Cord Derived from Intrathecal Administration of Stem Cells (P4.234)
CONCLUSIONS: Unregulated intrathecal SCT can cause devastating complications. This patient’s highly proliferative neoplasm developing from pluripotent stem cells supports the ideological shift to employ more differentiated cells in future SCT research.Disclosure: Dr. Saad has nothing to disclose. Nursing Home Residents with Seizure Disorders/Epilepsy: Prevalence, Characteristics, and Treatment Patterns (P1.080)
Conclusions: NHRs with seizure disorder/epilepsy have substantial physical and cognitive impairment. Nearly half were prescribed combination AED therapy, and >20[percnt] took 蠅3 different AEDs, suggesting refractory epilepsy. Additional research is needed to assess treatment outcomes and further define optimal care in this population.Disclosure: Dr. Zarowitz has nothing to disclose. Dr. Allen has nothing to disclose. Association of Urinary Continence Recovery with Cognition, Transfers and Discharge Disposition Following Inpatient-Rehabilitation for Stroke (P3.301)
Conclusions: 51.6[percnt] stroke patients have complete UI on IR admission. UI recovered at-least partially in 61.5[percnt] by IR discharge; and was associated with greater functional-independence for transfers and cognition. ACTs and SNF dispositions are more likely in patients that remain UI on discharge. UI recovery to minimal-assistance to independent was associated with the most home/community dispositions (82[percnt]).Disclosure: Dr. Kushner has nothing to disclose. Incontinence and gait disturbance after intraventricular extension of intracerebral hemorrhage
Conclusion: ICH subjects with IVH extension are at an increased risk for developing incontinence and dysmobility after controlling for factors associated with severity and disability. This finding suggests a potential target to prevent or treat long-term disability after ICH with IVH.

Teaching NeuroImages: Longitudinally extensive transverse myelitis in MELAS
A 16-year-old girl presented with acute-onset flaccid quadriparesis with urinary incontinence. Medical history unveiled generalized epilepsy since age 7 years and status epilepticus 3 years before. Multisystem compromise was absent. Family history was unremarkable. Two days after admission, generalized refractory status epilepticus occurred. Neuroimaging studies disclosed laminar cortical necrosis and longitudinally extensive transverse myelitis (LETM) in cervical and thoracic spinal cord levels (figure). Muscle biopsy disclosed subsarcolemmal mitochondrial proliferation and ragged-red fibers (figure). Genetic testing conf...

Change in cognitive performance is associated with functional recovery during post-acute stroke rehabilitation: a multi-centric study from intermediate care geriatric rehabilitation units of Catalonia
Abstract Recovery after a stroke is determined by a broad range of neurological, functional and psychosocial factors. Evidence regarding these factors is not well established, in particular influence of cognition changes during rehabilitation. We aimed to investigate whether selective characteristics, including cognitive performance and its change over time, modulate functional recovery with home discharge in stroke survivors admitted to post-acute rehabilitation units. We undertook a multicenter cohort study, including all patients discharged from acute wards to any geriatric rehabilitation unit in Catalonia-Spai...

Prevalence and associated factors of anger post stroke: a systematic review.
CONCLUSION: There is a high prevalence of anger post stroke. Psychiatric factors have been associated to it, while other associations are less conclusive. To improve anger knowledge and management, it would be necessary to improve its definition and assessment. PMID: 26005071 [PubMed - in process]

Bilateral Frontal Cerebral Infarctions Presenting as Ascending Weakness: Case Report (P1.023)
CONCLUSIONS: To our knowledge there are only a few case reports describing bilateral frontal cerebral infarctions due to a single A2 segment of the anterior cerebral artery. Bilateral cerebral infarctions should be considered in patients with symmetric weakness, especially in those with vascular risk factors who do not have features consistent with an alternative etiology. Reflexes are an important feature in distinguishing between central and peripheral lesions and should be closely monitored in these patients. Timely diagnosis will enable early delivery of thrombolytic therapy to these patients improving outcomes.Disclos...

Demographic Characteristics and Prognostic Factors in Acute Ischemic Stroke in a Tertiary Care Hospital in Nepal (P1.073)
