Influence of renal impairment on clinical outcomes after endovascular recanalization in vertebrobasilar artery occlusions
Conclusions Our findings suggest that RI is associated with a higher risk of sICH in hospital and a decrease in survival, favorable functional outcome, and functional improvement at 90 days and 1 year follow-up. Machine learning and acute stroke imaging
Conclusions ML in acute ischemic stroke imaging has already made tremendous headway. Additional applications and further integration with clinical care is inevitable. Thus, facility with these approaches is critical for the neurointerventional clinician.

Association between time to treatment and clinical outcomes in endovascular thrombectomy beyond 6 hours without advanced imaging selection
Conclusion In this real-world study, there was minimal change in functional disability, sICH and in-hospital mortality within and across the late window. While confirmatory randomized trials are needed, these findings suggest that EVT remains feasible and safe when performed in AIS patients selected without advanced neuroimaging between 6–24 hours from stroke onset.

Poor clinical outcome despite successful basilar occlusion recanalization in the early time window: incidence and predictors
Conclusions Early successful reperfusion with EVT occurred in 80.5% of patients, and the only treatment-related factor predictive of clinical outcome was first pass mTICI 2b-3 reperfusion. Further research is warranted to identify the optimal techniques and devices associated with first pass reperfusion in the posterior circulation.

E-051 Regional differences in infarction among stroke patients with low aspects: a retrospective cohort study
ConclusionBrain regions are differentially affected in patients with low ASPECTS, but whether this variability contributes to long-term outcomes and should guide treatment decisions requires further investigation with a larger sample size.Abstract E-051 Figure 1Disclosures M. E-108 Endovascular treatment of patients with acute ischemic stroke due to underlying arterial dissection: a nationwide analysis
ConclusionsDespite a similar prevalence among patients admitted for AIS, CA dissections are associated with a higher rate of endovascular intervention and poorer short-term outcomes compared to VA dissections. E-224 Outcomes of mechanical thrombectomyin stroke patients with extreme large infarction core
ConclusionAbout one in five patients presenting with extreme large infarction core (ASPECTS 0-2) may achieve a favorable 90-day functional outcome following MT. However, MT in this group of patients was associated with high mortality and sICH rate.Disclosures E. Almallouhi: None. M. Anadani: None. S. Samir Elawady: None. M. Mahdi Sowlat: None. I. E-247 The universal effect of the 'golden hour on stroke thrombectomy outcomes
ConclusionsIn this work we demonstrate the universality of the ‘Golden Hour’ effect, in which procedures lasting more than 1 hour are associated with worse clinical outcomes and higher rates of symptomatic hemorrhage. This study will assist clinicians in determining when to be more aggressive in their approach when aiming for TICI 3 reperfusion in different subpopulations of patients.Disclosures M. Ash: None. L. Clinical, angiographic and radiographic outcome differences among mechanical thrombectomy devices: initial experience of a large-volume center
Conclusions Although our initial data confirm the superiority of SR technology over the Merci device, there was no significant difference in near complete/complete reperfusion, final infarct volumes or clinical outcomes between SR and Penumbra thromboaspiration.

Unprotected carotid artery stenting in symptomatic elderly patients: a single-center experience
Conclusions Endovascular treatment of carotid stenosis without protection devices in symptomatic patients aged >75 years is an alternative to endovascular treatment with protection devices. Complications and mortality rates are similar to studies that used protection devices in lower risk patients.

E-067 management of tandem intracranial and extracranial occlusion causing hyper acute ischemic stroke with extracranial cervical stenting in conjunction with mechanical thrombectomy
ConclusionsMajor Strokes due to TIM and VA origin/basilar occlusions can be successfully treated by extra-cranial internal carotid artery or vertebral artery stenting and mechanical thrombectomy (MT).Advances in knowledgeWe favor the use of a hybrid approach in terms of the proximal-distal sequence as it achieves early cerebral recanalization and also reduces the procedure time. This is one of the largest studies at present in the United Kingdom.DisclosuresS. E-095 what are the risk factors and outcomes relating to hemorrhagic transformation with large vessel occlusion in the anterior circulation at a comprehensive stroke center?
ConclusionMT is associated with an increased rate of HI hemorrhages but not PH hemorrhages. HI hemorrhages have no correlation with death or mRS. mRS may be too insensitive to detect deficits relating to HI hemorrhages, NIHSS should be used in studies investigating clinical outcomes relating to HI hemorrhages. BG involvement may correlate with both HI and PH hemorrhages in patients treated with and without MT. However, having BG involvement does not seem to correlate with a worse prognosis in MT patients with any type of hemorrhage.DisclosuresG. Bennett: None. M. Al Hasan: None. G. Vidal: 3; C; Penumbra.
