Influence of glucose levels on clinical outcome after mechanical thrombectomy for large-vessel occlusion: a systematic review and meta-analysis
Mechanical thrombectomy (MT) represents the mainstay of treatment for patients with acute ischemic stroke due to large-vessel occlusion (LVO). Intravenous thrombolysis has been associated with worse clinical outcome in patients presenting with high blood glucose levels at admission; to date the true effect of hyperglycemia in the setting of MT has not been fully elucidated. In this meta-analysis, we analyzed the influence of high blood glucose levels at admission on clinical outcome after MT. Ovid EMBASE, PubMed, Scopus, and Cochrane Library databases were searched from their dates of inception up to March 2021. An initial...

Direct endovascular treatment versus bridging therapy in patients with acute ischemic stroke eligible for intravenous thrombolysis: systematic review and meta-analysis
Conclusion Compared with bridging therapy, direct EVT may be equally effective and yield a lower rate of ICH and clot migration in patients with AIS. Trail registration number PROSPERO: CRD42021236691.

Effectiveness and safety of endovascular thrombectomy for large versus medium vessel occlusions: a single-center experience
Conclusion The 90-day mRS and rate of periprocedural complications were not significantly different between patients treated for LVO and those treated for MeVO with EVT. Older age, not receiving r-tPA, and larger infarct size were independent predictors of poor outcome at 90 days.

Embotrap Extraction & Clot Evaluation & Lesion Evaluation for NeuroThrombectomy (EXCELLENT) Registry design and methods
Conclusions The EXCELLENT registry will provide reproducible effectiveness and safety data of EmboTrap for its use for mechanical thrombectomy. Additionally, the study will characterize the blood clots retrieved during mechanical thrombectomy with respect to their composition and histopathological analysis and potential correlations with clinical and imaging findings. Trial registration number NCT03685578.

E-145 Collateral patterns in large vessel occlusion stroke and outcomes after endovascular thrombectomy
ConclusionsCollateral pattern is a robust determinant of 90-day outcomes after EVT. It may help guide EVT decisions in the delayed window, especially when there are delays to treatment such as those related to patient transfer. Further prospective studies are needed to evaluate the role of collateral pattern in treatment decisions and prognostication.Disclosures R. Regenhardt: None. M. Lev: None. J. He: None. A. E-222 How far is too far in stroke thrombectomy? from xl to xs vessel occlusions with radially adjustable stent-retrievers - tigertriever xl and tigertriever 13
ConclusionMechanical thrombectomy for both primary or secondary DMVO seems feasible and as safe as for LVO. Our initial experience using the Tigertriever13 is of special interest as it shows we can potentially significantly expand AIS population that can benefit from mechanical thrombectomy treatment.Disclosures A. Unexplained early neurological deterioration after endovascular treatment for acute large vessel occlusion: incidence, predictors, and clinical impact: Data from ANGEL-ACT registry
Conclusions ENDunexplained associated with poor 90day outcomes occurred in 4.3% of patients with acute LVO undergoing EVT. Several independent predictors of ENDunexplained were identified in this study, which should be considered in daily practice to improve acute LVO management. Diversion-p64: results from an international, prospective, multicenter, single-arm post-market study to assess the safety and effectiveness of the p64 flow modulation device
Conclusions Diversion-p64 is the largest prospective study using the p64 flow modulation device. The results of this study demonstrate that the device has a high efficacy and carries a low rate of mortality and permanent morbidity.
