P151/175 Direct aspiration with EMBOVAC - first clinical experience and clot composition (results the "PERFECT study)
ConclusionThis is the first clinical evaluation of EMBOVAC. Direct aspiration with EMBOVAC as first line therapy resulted in high rates of reperfusion and favorable clinical outcomes. Clot composition analysis can aid in better understanding of the stroke etiology and means for effective clot removal.Disclosure of InterestAcandis (OJ, CL, JF); Microvention (GM, CC, JF); Stryker (GM, CC, JF); Medtronic (GM, CC, JF); Surge2surgery (BG); MIVI (CC); Phenox (CL, JF); Penumbra (CL, JF); GSK (AS); German Ministry of Science & Education and Economy & Innovation, EU, Hamburgische Investitions-/Förderbank, Philips, B...

P152/178 Safety and efficiency of bridging i.v. thrombolysis in M2 occlusions: a propensity-score-matched registry cohort study
ConclusionIn M2-occlusions, MT+IVT was not associated with increased risk of sICH. Patients receiving MT+IVT had lower 90d-mRS and a lower increase in mRS pre-stroke to 90d. However, probability of functional independence and rates of successful recanalization were similar compared to matched controls.Disclosure of InterestHK has financial interest in Eppdata GmbH.GT received fees as consultant and lecturer from Acandis, Alexion, Amarin, Boehringer Ingelheim, Bayer, BMS/Pfizer, Daiichii Sankyo and Portola. P156/208 Association between arterial diameter and outcome in acute M2 occlusive ischemic stroke patients treated with endovascular thrombectomy
ConclusionThis study demonstrated that the lowest quartile of M2 diameter was associated with procedural complication in the acute M2 occlusion patients treated with EVT. However, clinical outcome was not differed.Disclosure of InterestNothing to disclose

P164/236 Antithrombotic regimen in emergent carotid stenting for acute ischemic stroke due to tandem lesion: a meta-analysis of aggregate data
ConclusionsIn AIS due to tandem occlusion, the periprocedural antithrombotic regimen of eCAS seems to have a marginal effect on good functional outcome. Overall, high intensity antithrombotic therapy may provide a marginal benefit on good functional outcome and carotid stent patency without a significant increase in risk of sICH.Disclosure of InterestNothing to disclose

P170/281 Endovascular thrombectomy with or without thrombolysis bridging in patients with acute ischemic stroke: a systematic review and meta-analysis of randomized trials
ConclusionLow-certainty evidence shows a small possible decrease in functional independence, low-certainty evidence shows a small possible decrease in mortality, and moderate-certainty evidence that there is probably a decrease in sICH for patients with EVT alone compared to EVT plus IVT.Disclosure of InterestThe authors have nothing to disclose.

P174/304 CGuard stent in emergent settings- is the dual-layer stent safe? A multicenter study
ConclusionIn this multicenter study, the use of CGuard for emergent carotid artery stenting, particularly in tandem occlusions, was safe and resulted in lower rates of in-stent occlusions.Disclosure of InterestThe authors stated explicitly that there are no conflicts of interest in connection with this abstract.

Caveolin-1 aggravates neurological deficits by activating neuroinflammation following experimental intracerebral hemorrhage in rats
CONCLUSIONS: Our research revealed CAV1 aggravated neurological dysfunction in a rat ICH model. CAV1 knockdown exerted neuroprotective effect by suppressing microglia activation and neuroinflammation after ICH might via the C-Src/CAV1/IKK-β/NF-κB signaling pathway.PMID:37598879 | DOI:10.1016/j.expneurol.2023.114508

Improving outcomes in intracerebral hemorrhage through microglia/macrophage-targeted IL-10 delivery with phosphatidylserine liposomes
Biomaterials. 2023 Aug 15;301:122277. doi: 10.1016/j.biomaterials.2023.122277. Online ahead of print.ABSTRACTIntracerebral hemorrhage (ICH) remains the most lethal type of stroke, and effective clinical therapies that can speed up hematoma resolution after ICH are still lacking. While the beneficial effects of IL-10 on ICH recovery have been demonstrated, the clinical translation of IL-10 requires effective delivery methods by which sufficient IL-10 can be delivered to ICH-affected regions in the brain. Here we report the use of a phosphatidylserine (PS) liposome (PSL)-based nanoparticle system for microglia/macrophage-tar...

Acute ischemic stroke: The role of emergency carotid endarterectomy in isolated extracranial internal carotid artery occlusion
CONCLUSIONS: Emergency CEA in isolated eICA occlusion has proved to be a safe and effective treatment option in selected patients. CT perfusion, imaging the ischemic penumbra and quantifying the tissue suitable for reperfusion, offers a valid support in the diagnostic-therapeutic workup. Indeed, we can infer that the area of the ischemic penumbra is directly proportional to the margin of clinical improvement after revascularization, supposing that the appropriate intervention timing is respect.PMID:37594376 | DOI:10.1177/17085381231192712
